分析万古霉素联合头孢呋辛加局部庆大霉素浸渍胶原海绵预防脊柱骨折手术感染的效果  

Analysis of the effect of vancomycin combined with cefuroxime and local gentamicin impregnated collagen sponge in preventing spinal surgery infection

摘　　要：目的分析局部庆大霉素浸渍胶原海绵+万古霉素+头孢呋辛对脊柱骨折手术感染的预防价值。方法以120例脊柱骨折手术患者为样本,按照单双号法分为一般组、分析组,每组60例。一般组使用局部庆大霉素浸渍胶原海绵,分析组在一般组基础上加用头孢呋辛+万古霉素。对比两组炎症因子水平、感染率、并发症发生率、不良反应发生率、手术愈合质量及生活质量评分。结果用药前,两组肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)、C反应蛋白(CRP)水平对比差异性较小(P>0.05)。用药后,分析组TNF-α、IL-6、CRP水平分别为(42.17±3.96)ng/L、(170.34±15.68)pg/ml、(8.85±1.40)mg/L,一般组分别为(50.89±4.53)ng/L、(208.91±17.24)pg/ml、(13.11±2.76)mg/L,两组TNF-α、IL-6、CRP水平均降低,且分析组低于一般组(P<0.05)。分析组感染率为3.33%,一般组为16.67%,分析组低于一般组(P<0.05)。分析组并发症发生率为5.00%,一般组为18.33%,分析组低于一般组(P<0.05)。分析组不良反应发生率为3.33%,一般组为6.67%,两组不良反应发生率对比差异性较小(P>0.05)。分析组手术愈合质量优于一般组(P<0.05)。用药后,分析组功能、角色、疼痛、健康、社会、情感、心理、活力评分分别为(89.15±4.03)、(85.34±4.18)、(86.63±4.45)、(87.10±4.98)、(89.75±4.22)、(88.54±5.01)、(86.95±4.77)、(87.55±4.04)分,高于一般组的(83.62±4.71)、(79.66±5.42)、(80.75±4.98)、(80.66±5.31)、(82.63±4.64)、(82.79±4.02)、(80.12±4.56)、(81.23±4.94)分(P<0.05)。结论局部庆大霉素浸渍胶原海绵+万古霉素+头孢呋辛的方案预防效果好,能改善脊柱骨折手术后的炎症问题,减少感染事件以及其他并发症发生风险,提升手术愈合质量,改善术后生活。Objective To analyze the preventive value of local gentamicin impregnated collagen sponge+vancomycin+cefuroxime for spinal surgery infection.Methods A total of 120 patients undergoing spinal surgery were selected as samples and divided into a general group and an analysis group according to odd-even number method,with 60 cases in each group.The general group was treated with local gentamicin impregnated collagen sponge,while the analysis group was treated with cefuroxime+vancomycin based on the treatment of general group.The levels of inflammatory factors,infection rate,incidence of complications,incidence of adverse reactions,quality of healing and quality of life score were compared between the two groups.Results Before medication,the levels of tumor necrosis factor-α(TNF-α),interleukin-6(IL-6)and C-reactive protein(CRP)were compared,and the differences was small between the two groups(P>0.05).After medication,the levels of TNF-α,IL-6 and CRP of the analysis group were(42.17±3.96)ng/L,(170.34±15.68)pg/ml and(8.85±1.40)mg/L,while those of the general group were(50.89±4.53)ng/L,(208.91±17.24)pg/ml and(13.11±2.76)mg/L.The levels of TNF-α,IL-6 and CRP in both groups were decreased,and the analysis group was lower than the general group(P<0.05).The infection rate was 3.33%in the analysis group and 16.67%in the general group,and the analysis group was lower than the general group(P<0.05).The incidence of complications was 5.00%in the analysis group and 18.33%in the general group,and the analysis group was lower than the general group(P<0.05).The incidence of adverse reactions was 3.33%in the analysis group and 6.67%in the general group,and there was little difference between the two groups(P>0.05).The quality of healing in the analysis group was better than that in the general group(P<0.05).After medication,the function,role,pain,health,social,emotional,psychological,and vitality scores in the analysis group were(89.15±4.03),(85.34±4.18),(86.63±4.45),(87.10±4.98),(89.75±4.22),(88.54±5.01),(86.95±4.77
