经腹部与经阴道超声诊断早期异位妊娠的效能比较

摘　　要：目的探讨经腹部与经阴道超声诊断早期异位妊娠的效能。方法87例疑似早期异位妊娠患者,均实施经腹部与经阴道超声检查。以手术病理结果为金标准,记录两种超声检查结果与金标准结果,比较两种超声检查对早期异位妊娠的诊断效能、典型超声影像学征检出率。结果金标准中,阳性53例,阴性34例;经腹部超声中,阳性51例(真阳性43例,假阳性8例),阴性36例(真阴性26例,假阴性10例);经阴道超声中,阳性52例(真阳性52例,假阳性0例),阴性35例(真阴性34例,假阴性1例)。相较于经腹部超声的81.13%、76.47%、79.31%、84.31%、72.22%,经阴道超声诊断早期异位妊娠的敏感度98.11%、特异度100.00%、准确度98.85%、阳性预测值100.00%与阴性预测值97.14%均显著更高(P<0.05)。相较于经腹部超声(30.77%、59.26%、45.83%、76.92%、53.85%),经阴道超声对子宫内假孕囊、原始心血管搏动、胚芽、附件包块以及盆腔积液的检出率(100.00%、92.59%、91.67%、98.08%、92.31%)更高(P<0.05)。结论与经腹部超声检查相比,经阴道超声诊断早期异位妊娠的效能更高,对典型超声影像学征检出率高,有助于指导临床早期诊治。Objective To explore the efficacy of transabdominal and transvaginal ultrasound in diagnosing early ectopic pregnancy.Methods Transabdominal and transvaginal ultrasonography were performed in 87 patients suspected of early ectopic pregnancy.With surgical pathology as the gold standard,two types of ultrasound results and gold standard results were recorded,and the diagnostic efficiency and the detection rate of typical ultrasound imaging signs of the two types of ultrasound in early ectopic pregnancy were compared.Results In the gold standard,53 cases were positive and 34 cases were negative;in transabdominal ultrasound,51 cases were positive(43 true positive cases,8 false positive cases),36 cases were negative(26 true negative cases,10 false negative cases);in transvaginal ultrasound,52 cases were positive(52 true positive cases,0 false positive cases),35 cases were negative(34 true negative cases,1 false negative case).Compared with 81.13%,76.47%,79.31%,84.31%,and 72.22%of transabdominal ultrasound,the sensitivity,specificity,accuracy,positive predictive value,and negative predictive value of transvaginal ultrasound in early ectopic pregnancy were significantly higher at 98.11%,100.00%,98.85%,100.00%,and 97.14%(P<0.05).Compared with transabdominal ultrasound(30.77%,59.26%,45.83%,76.92%,53.85%),transvaginal ultrasound has a higher detection rate(100.00%,92.59%,91.67%,98.08%,92.31%)for intrauterine false gestational sac,primitive cardiovascular pulsation,embryo,adnexal mass,and pelvic effusion(P<0.05).Conclusion Compared with transabdominal ultrasound,transvaginal ultrasound has a higher diagnostic efficacy for early ectopic pregnancy,with a higher detection rate for typical ultrasound imaging signs,which helps guide early clinical diagnosis and treatment.
