米非司酮联合米索前列醇在早期稽留流产患者中的应用研究

摘　　要：目的探讨米非司酮联合米索前列醇在早期稽留流产患者中的应用效果。方法研究对象为98例早期稽留流产患者,参照随机数字表分为对照组(49例)和研究组(49例)。对照组患者采用米索前列醇治疗,研究组患者采用米非司酮联合米索前列醇治疗。比较两组患者的流产效果、清宫率、妊娠囊排出时间、阴道出血量、阴道出血时间、月经恢复时间、血β-人绒毛膜促性腺激素(β-hCG)水平以及用药不良反应发生率。结果研究组流产效果及清宫率均优于对照组(P<0.05)。研究组阴道出血量(87.50±3.56)ml较对照组的(95.45±4.00)ml少,妊娠囊排出时间(3.32±0.98)h、阴道出血时间(8.12±1.40)d、月经恢复时间(34.02±2.55)d较对照组的(6.20±1.22)h、(11.02±1.60)d、(38.45±3.85)d短(P<0.05)。治疗前,两组血β-hCG水平比较未见差异性(P>0.05);治疗后,两组血β-hCG水平均较治疗前显著下降,且研究组血β-hCG(755.05±112.05)mIU/ml较对照组的(1000.60±120.88)mIU/ml更低(P<0.05)。两组用药不良反应发生率比较未见差异性(P>0.05)。结论米非司酮联合米索前列醇能够有效提高早期稽留流产患者的完全流产率,改善β-hCG水平,降低清宫率,且患者的阴道出血量少,康复速度快,用药安全性较佳。Objective To explore the application effect of mifepristone combined with misoprostol in patients with early missed abortion.Methods 98 patients with early missed abortion as study subjects were divided into control group(49 cases)and study group(49 cases)according to the random numerical table.The control group was treated with misoprostol,and the study group was treated with mifepristone combined with misoprostol.Comparison of abortion effect,curettage rate,time to expulsion of the gestational sac,vaginal bleeding amount,vaginal bleeding time,menstrual recovery time,bloodβ-human chorionic gonadotropin(β-hCG)level and the incidence of adverse drug reactions between the two groups.Results The abortion effect and curettage rate of the study group were better than those of the control group(P<0.05).The study group had a less vaginal bleeding amount of(87.50±3.56)ml than(95.45±4.00)ml in the control group;in the study group,the time to expulsion of the gestational sac was(3.32±0.98)h,the vaginal bleeding time was(8.12±1.40)d and the menstrual recovery time was(34.02±2.55)d,which were shorter than(6.20±1.22)h,(11.02±1.60)d and(38.45±3.85)d in the control group(P<0.05).Before treatment,there was no difference in bloodβ-hCG level between the two groups(P>0.05).After treatment,the bloodβ-hCG level in both groups was significantly decreased compared with before treatment,and the study group had a lower bloodβ-hCG of(755.05±112.05)mIU/ml than(1000.60±120.88)mIU/ml in the control group(P<0.05).There was no difference in the incidence of adverse drug reactions between the two groups(P>0.05).Conclusion Mifepristone combined with misoprostol can effectively increase the complete abortion rate,improve the level ofβ-hCG and reduce the curettage rate in patients with early missed abortion.Moreover,the patient has less vaginal bleeding,faster recovery and better medication safety.
