芩连清胆汤联合四联疗法治疗幽门螺杆菌相关胃炎(胆热犯胃证)的临床疗效

摘　　要：目的分析幽门螺杆菌相关胃炎(胆热犯胃证)患者使用芩连清胆汤联合四联疗法治疗的效果。方法38例幽门螺杆菌相关胃炎(胆热犯胃证)患者,选择随机数字表法分为常规组和研究组,各19例。常规组患者使用四联疗法治疗,研究组使用四联疗法加用芩连清胆汤治疗。比较两组患者幽门螺杆菌根除率、中医证候积分及胃蛋白酶原Ⅰ水平、胃蛋白酶原Ⅱ水平、胃泌素水平、胃动素水平。结果研究组幽门螺杆菌根除率为84.21%,高于常规组的52.63%,组间差异性显著(P<0.05)。治疗后,研究组胃蛋白酶原Ⅰ、Ⅱ及胃泌素、胃动素水平分别为(132.69±12.21)μg/L、(10.32±1.14)μg/L、(98.89±1.16)ng/L、(169.07±14.07)ng/L,均优于常规组的(106.03±11.15)μg/L、(13.02±1.08)μg/L、(106.60±1.08)ng/L、(192.37±15.12)ng/L,组间差异性显著(P<0.05)。研究组痞满、胃脘痛、嘈杂、吐酸积分均低于常规组,组间差异性显著(P<0.05)。结论幽门螺杆菌相关胃炎(胆热犯胃证)患者使用芩连清胆汤联合四联疗法进行治疗,可改善患者胃蛋白酶水平,提升幽门螺杆菌根除率,降低患者中医证候积分,临床应用价值较高。Objective To analyze the effect of Qinlian Qingdan Decoction combined with quadruple therapy in the treatment of patients with Helicobacter pylori related gastritis(gallbladder heat attacking stomach syndrome).Methods 38 patients with Helicobacter pylori related gastritis(gallbladder heat attacking stomach syndrome)were divided into conventional group and study group by random numerical table,with 19 cases in each group.The conventional group was treated with quadruple therapy,and the study group was treated with quadruple therapy plus Qinlian Qingdan Decoction.The eradication rate of Helicobacter pylori,traditional Chinese medicine syndrome score and levels of pepsinogenⅠ,pepsinogenⅡ,gastrin and motilin were compared between the two groups.Results The eradication rate of Helicobacter pylori in the study group was 84.21%,which was higher than 52.63%in the conventional group,and the difference between groups was significant(P<0.05).After treatment,the levels of pepsinogenⅠ,Ⅱ,gastrin and motilin in the study group were(132.69±12.21)μg/L,(10.32±1.14)μg/L,(98.89±1.16)ng/L and(169.07±14.07)ng/L,which were better than(106.03±11.15)μg/L,(13.02±1.08)μg/L,(106.60±1.08)ng/L and(192.37±15.12)ng/L in the conventional group,and the difference between groups was significant(P<0.05).The scores of distention and fullness,epigastric pain,noise and acid regurgitation in the study group lower than the conventional group,and the difference between groups was significant(P<0.05).Conclusion The use of Qinlian Qingdan Decoction combined with quadruple therapy in the treatment of patients with Helicobacter pylori related gastritis(gallbladder heat attacking stomach syndrome)can improve the level of pepsin,increase the eradication rate of Helicobacter pylori,and reduce the scores of traditional Chinese medicine syndrome of patients,which has high clinical application value.
