鹿茸联合克罗米芬对DOR不孕患者促排卵的影响研究

摘　　要：目的研究鹿茸联合克罗米芬对卵巢储备功能减退(DOR)不孕患者促排卵的临床疗效及安全性。方法选取60例DOR不孕患者,根据随机数字表法分为对照组和治疗组,每组30例。对照组给予克罗米芬片治疗,治疗组给予克罗米芬片联合鹿茸治疗。比较两组患者的血清激素水平、周期排卵率、妊娠率、中医证候积分、临床疗效及不良反应发生率。结果治疗后,治疗组周期排卵率(36.84±5.09)%、妊娠率13.33%均高于对照组的(17.63±3.28)%、0(P<0.05)。治疗1个疗程后,治疗组中医证候主证积分、中医证候次证积分及中医证候总积分分别为(5.65±1.34)、(6.83±1.51)、(12.48±2.35)分,均低于对照组的(6.61±1.57)、(7.92±1.68)、(14.53±2.46)分(P<0.05)。治疗组总有效率80.00%高于对照组的53.33%(P<0.05)。治疗1个疗程后,治疗组患者血清孕酮、雌二醇水平高于对照组,血清促黄体生成激素、卵泡刺激素水平低于对照组(P<0.05)。两组不良反应发生率比较无差异(P>0.05)。结论在克罗米芬片治疗基础上加用鹿茸,能够进一步促进DOR不孕患者排卵,提高患者的妊娠率及临床疗效,改善激素水平,且未增加卵巢过度刺激征等发生,值得临床重视。Objective To study the clinical efficacy and safety of pilose antler combined with clomiphene on ovulation promotion in infertile patients with diminished ovarian reserve(DOR).Methods 60 infertile patients with DOR were selected and divided into control group and treatment group according to random numerical table,each consisting of 30 cases.The control group was treated with clomiphene tablets,and the treatment group was treated with clomiphene tablets combined with pilose antler.Comparison of serum hormone levels,ovulation rate,pregnancy rate,traditional Chinese medicine syndrome score,clinical efficacy and incidence of adverse reactions between the two groups.Results After treatment,the ovulation rate and pregnancy rate of the treatment group were(36.84±5.09)%and 13.33%,which were higher than(17.63±3.28)%and 0 in the control group(P<0.05).After 1 course of treatment,the main syndrome score,secondary syndrome score and total syndrome score in the treatment group were(5.65±1.34),(6.83±1.51)and(12.48±2.35)points,which were lower than(6.61±1.57),(7.92±1.68)and(14.53±2.46)points in the control group(P<0.05).The total effective rate of 80.00%in the treatment group was higher than 53.33%in the control group(P<0.05).After 1 course of treatment,the serum levels of progesterone and estradiol in the treatment group were higher than those in the control group,and the serum levels of luteinizing hormone and follicle-stimulating hormone were lower than those in the control group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion On the basis of clomiphene tablets,the addition of pilose antler can further promote ovulation in infertile patients with DOR,improve the pregnancy rate and clinical efficacy of patients,improve hormone levels,and do not increase the occurrence of ovarian hyperstimulation,which is worthy of clinical attention.
