摘　　要：目的 探讨隧道式外周置管中心静脉导管(PICC)在传统PICC置管困难患者中的应用效果。方法 将2023年1月—2024年8月本院收治的PICC置管困难的31例恶性肿瘤患者纳入研究组,将308例常规PICC置管患者纳入对照组,比较两组的一次性穿刺成功率、置管成功率、操作时间、非计划拔管率、置管疼痛、手臂舒适度及并发症发生率。结果 研究组与对照组在一次性穿刺成功率和置管成功率方面的差异无统计学意义(P>0.05),但相较于对照组,研究组操作时间较长,疼痛评分较低,非计划拔管率低,手臂舒适度评分较高(P<0.05)。研究组并发症发生率为3.23%,对照组为16.89%,差异具有统计学意义(P<0.05)。结论 隧道式PICC对于PICC置管困难的患者安全可行,具有较高的舒适度和较低的并发症发生率。Objective To explore the clinical application effect of tunnel PICC.Methods 31 patients with malignant tumors with difficulty in PICC placement between January 2023 and August 2024 were selected as the study group,and 308 patients with conventional PICC placement were selected as the control group.The one-time piercing success rate,tube success rate,operation time,unplanned exculation rate,tube pain,arm comfort and complication rates were Results There was no significant difference between the study group and the control group in the one-time piercing success rate and placement success rate(P>0.05),but the operation time and pain score were high,the unplanned exfoliation rate was low,and the arm comfort score was high(P<0.05).The incidence of complications in the study group was 3.23%and 16.89%in the control group,with significant differences(P<0.05).Conclusion Tunnel PICC is safe and feasible for patients with difficulty in PICC placement,with high comfort and low incidence of complications.
