4C护理模式在先天性肛门闭锁儿童的围手术期护理中的应用体会

摘　　要：目的 总结4C护理模式在1例先天性肛门闭锁患儿围手术期中应用的体会。方法 选择本院收治的1例先天性肛门闭锁儿童,运用4C护理模式,构建4C护理干预小组进行全面性、合作性、协调性、延续性干预。结果 经过对症治疗及针对性护理后,患儿取得了期望的治疗效果并顺利出院。患儿住院11.6 d,未发生并发症,本例患儿干预后肛门功能(大便控制、污粪、肛门括约肌收缩、总分)高于干预前。干预后患儿的生活质量评分(躯体功能、情感功能、社交功能)高于干预前。结论 4C护理模式在先天性肛门闭锁手术患儿的护理中得到应用,其服务领域不仅限于医院内,还可以向医院外部拓展。4C护理模式致力于提供全方位、协作、协调和连续的护理服务,最大限度地为先天性肛门闭锁患儿提供持续且高品质的护理,使患儿的护理需求得到充分的满足,并推动医疗保健工作得到进一步发展。Objective To summarize the nursing experience of applying the 4C nursing model during the perioperative period of a child with congenital anal atresia,and emphasize the importance of applying the 4C nursing model in children with congenital anal atresia in China.Methods A child with congenital anal atrexia admitted to our hospital was selected and a 4C nursing intervention group was established to conduct comprehensive,cooperative,coordinated and continuous intervention using 4C nursing model.Results Through the symptom therapy and specific care,the patient got the expected result and got out of the hospital.The duration of hospital stay is 11.6 days,and no complications occurred.The results showed that the anus function(bowel control,feces pollution,anal sphincter contraction and overall score)were significantly increased.The quality of life scores(physical function,emotional function,social function)of the children were higher after intervention than before intervention.Conclusion The 4C nursing model is adopted for intervention in the nursing process of children with congenital anal atresia surgery.Its service scope can be extended beyond the hospital,and through comprehensive,cooperative,coordinated,and continuous nursing services,continuous high-quality nursing services can be provided to children to the greatest extent possible,meeting the nursing needs of patients and promoting the improvement of medical and health services.Adopting the 4C nursing model for the perioperative period of children with congenital anal atresia can reduce the risk of postoperative complications,improve anal function,and it is worth to promote and apply it in clinic.
