摘　　要：目的 探讨实验室流感病毒检测中应用胶体金免疫层析技术、实时荧光定量PCR法的价值。方法 采集涟水辖区2019年4月—2023年10月医疗机构发热门诊2152份咽拭子样本,均实施胶体金免疫层析技术、荧光定量PCR法检测,以淮安市疾病预防控制中心复核确证结果为金标准,分析两种方案的检出情况、诊断效能。结果 淮安市疾病预防控制中心复核确证阳性样本315份,胶体金免疫层析技术检出阳性样本289份、荧光定量PCR法检测检出阳性样本311份。荧光定量PCR法检测灵敏度98.73%、准确率99.58%、特异度为99.73%,与金标准一致性(kappa值)为0.886,均高于胶体金免疫层析技术(P<0.05)。荧光定量PCR法对甲流/乙流检出率、与金标准符合率高于胶体金免疫层析技术(P<0.05)。结论 流感病毒检测中应用胶体金免疫层析技术、荧光定量PCR法均有一定价值,但荧光定量PCR检测法准确率、灵敏度更高,且与金标准有极高符合率,值得借鉴。Objective To explore the value of colloidal gold immunochromatography and realtime fluorescence quantitative PCR in laboratory influenza virus detection.Methods 2152 throat swab samples were collected from fever clinics in medical institutions in Lianshui area from April 2019 to October 2023.Colloidal gold immunochromatography and fluorescence quantitative PCR were used for detection.The results confirmed by the Huai'an Center for Disease Control and Prevention were used as the gold standard to analyze the detection and diagnostic efficacy of the two schemes.Results 315 positive samples were confirmed by the Huai'an Center for Disease Control and Prevention,289 positive samples were detected by colloidal gold immunochromatography,and 311 positive samples were detected by fluorescence quantitative PCR.The fluorescence quantitative PCR method has a sensitivity of 98.73%,an accuracy of 99.58%,a specificity of 99.73%,and a kappa value consistent with the gold standard of 0.886,all of which are higher than colloidal gold immunochromatography technology,with P< 0.05.The detection of H1N1 and H1N1 influenza by fluorescence quantitative PCR and its compliance with gold standards were higher than those by colloidal gold immunochromatography,with P< 0.05.Conclusion The application of colloidal gold immunochromatography technology and fluorescence quantitative PCR method in influenza virus detection have certain value,but fluorescence quantitative PCR detection method has higher accuracy and sensitivity,and has a high consistency rate with the gold standard,which is worth learning from.
