摘　　要：目的 探析董氏奇穴针刺治疗改善良性阵发性位置性眩晕(BPPV)患者行手法复位后残余症状的临床效果。方法 选取到杨浦区中医院针灸科门诊就诊的120例耳石症患者,按随机数字分组法将其分为对照组和治疗组,每组60例,均给予手法复位后口服倍他司汀基础治疗,对照组未给予其他治疗,治疗组在基础治疗后增加董氏奇穴针刺治疗,对比两组患者接受治疗前后头晕不稳等相关主观感觉的变化,以及治疗1~3个月后的复发情况。结果 两组患者复位成功后24 h、治疗后10 d的DHI评分均有降低,且治疗组的DHI评分明显低于对照组(P<0.05);治疗组患者治疗期间昏沉、头晕、走路不稳、漂浮感等残余症状发生频率及持续时间均明显低于对照组(P<0.05);两组患者经治疗1个月后耳石症复发情况对比差异无统计学意义(P>0.05),治疗组患者治疗3个月后耳石症复发情况明显低于对照组(P<0.05)。结论 对BPPV患者采用董氏奇穴针刺治疗,有助于缓解患者在手法复位后所残留的症状及其持续时间,有效减少复发率,从而提高患者的生活质量。Objective To explore the clinical effect of Dong's acupuncture treatment on the residual symptoms of benign paroxysmal positional vertigo (BPPV) patients after manual reduction.Methods 120 patients with otolith who went to the acupuncture and moxibustion Department of Yangpu District Hospital of Traditional Chinese Medicine were selected and divided into the control group and the treatment group according to the random number grouping method.60 patients in each group were given the basic treatment of oral betahistine after manual repositioning.The control group was not given other treatment.The treatment group added Dong's Qi point acupuncture after the basic treatment.The changes of subjective feelings related to dizziness and instability before and after treatment were compared between the two groups,as well as the recurrence after 1~3 months of treatment.Results The DHI scores of both groups of patients decreased 24 hours after successful reduction and 10 days after treatment,and the DHI scores of the treatment group were significantly lower than those of the control group at 24 hours after successful reduction and 10 days after treatment (P<0.05);The frequency and duration of residual symptoms such as drowsiness,dizziness,unsteady walking,and floating sensation in the treatment group were significantly lower than those in the control group during the treatment period (P<0.05);There was no significant difference in the recurrence of otolith between the two groups of patients after one month of treatment (P>0.05),while the recurrence of otolith in the treatment group was significantly lower than that in the control group after three months of treatment (P<0.05).Conclusion In the face of BPPV patients,our hospital's treatment with Dong's Qi acupoint can help improve the residual symptoms and their duration after manual reduction,reduce the recurrence rate,and thus improve their daily quality of life.
