摘 要:目的 分析不同剂量、不同部位欣母沛对腰硬联合麻醉剖宫产术产妇子宫收缩力及心血管系统稳定性的影响。方法 选取2021年7—12月本院腰硬联合麻醉下行剖宫产术产妇40例,依据欣母沛注射剂量分为A组(欣母沛250 μg,n=20)与B组(欣母沛500 μg,n=20),另外选取同期80例产妇,根据注射欣母沛部位不同分为臀大肌组(n=40)与宫体注射组(n=40)。对比不同组别注射欣母沛后1 min(T1)、15 min(T2)产妇脉搏血氧饱和度(SpO_(2))、每搏输出量(SV)及总外周阻力(SVR)变化。比较各组产妇子宫收缩情况,同时记录不良反应情况。结果 B组产妇T2时刻SV高于A组,SVR低于A组;注射5 min后子宫收缩功能评分高于A组(P<0.05)。宫体组产妇T2时刻SV高于臀大肌组,SVR低于臀大肌组(P<0.05);注射5 min后子宫收缩功能评分高于臀大肌组(P<0.05)。结论 高剂量欣母沛与宫体注射方式有利于提高剖宫产产妇心血管系统稳定性与增强子宫收缩功能。Objective To analyze the effects of different doses and different parts of Xinmupei on uterine contractility and cardiovascular system stability in cesarean section women undergoing lumbar epidural anesthesia.Methods Forty parturients undergoing cesarean section under lumbar epidural anesthesia in our hospital from July 2021 to December 2021 were selected.They were divided into Group A (250 μg,n=20) and Group B (500 μg,n=20) based on the injection dose of Xinmupei.Eighty parturients during the same period were selected and divided into the gluteus maximus group (n=40) and the intrauterine injection group (n=40) based on the different injection sites.Compare the changes in pulse oxygen saturation (SpO_2),output per beat (SV),and total peripheral resistance (SVR) of parturients at 1 minute (T1) and 15 minutes (T2) after injection of Xinmupei in different groups.Compare the uterine contractions of each group of parturients and record the incidence of adverse reactions in parturients.Results The cardiovascular system stability and uterine contraction ability of patients in group B were better than those in group A,and the incidence of adverse reactions was lower than that in group A (both P<0.05).The cardiovascular system stability and uterine contraction ability of patients in the uterine injection group were better than those in the gluteus maximus group,and the incidence of adverse reactions was lower than that in the gluteus maximus group (both P<0.05).Conclusion Intrauterine injection and increasing the dose of Xinmupei are beneficial for improving cardiovascular system stability and uterine contraction function in cesarean section women.
