老年全膝关节置换术中超声辅助股神经阻滞结合浸润麻醉效果及对疼痛程度、认知功能、苏醒质量的影响

摘　　要：目的 研究老年全膝关节置换术实施超声辅助股神经阻滞结合浸润麻醉的效果。方法 选择2021年1月—2023年12月在本院接受治疗的全膝关节置换术老年患者88例,并采取随机分组法分组,每组44例患者。其中,对照组实施超声辅助股神经阻滞,研究组实施超声辅助股神经阻滞结合浸润麻醉。对比两组自主呼吸能力复常、完全苏醒、苏醒室停留、术后住院总时间、术后不同时间点疼痛程度、认知功能评分、应激反应相关指标、麻醉效果、麻醉不良反应发生率。结果 研究组自主呼吸能力复常、完全苏醒、苏醒室停留、术后住院总时间短于对照组(P<0.05);研究组术后不同时间点疼痛程度、认知功能评分、应激反应相关指标改善幅度优于对照组(P<0.05);研究组麻醉效果优于对照组(P<0.05);研究组麻醉不良反应发生率低于对照组(P<0.05)。结论 老年全膝关节置换术患者,接受超声辅助股神经阻滞结合浸润麻醉效果明显。Objective To investigate the effect of ultrasound assisted femoral nerve block combined with infiltration anesthesia for elderly patients with total knee arthroplasty.Methods The paper chose 88 elderly patients with total knee arthroplasty in our hospital from January 2021 to December 2023,and divided them into two groups randomly,with 44 cases in each group.The control group was treated with ultrasound assisted femoral nerve block,and the study group with ultrasound assisted femoral nerve block combined with infiltration anesthesia.Recovery of spontaneous breathing ability,complete recovery,stay in the recovery room,total postoperative hospital stay,pain levels at different postoperative time points,cognitive function scores,stress response related indicators,anesthesia effect and incidence of adverse anesthesia reactions were compared between two groups.Results Self-directed breathing ability,complete recovery,stay in the recovery room and total postoperative hospital stay in the study group was shorter than the control group,data between the groups were comparable,(P<0.05).The study group had better improvement in pain levels,cognitive function scores and stress response related indicators at different time points after surgery than the control group,data were comparable between groups,(P<0.05).The anesthesia effect of the study group was better than the control group,data comparison was statistically significant between groups (P<0.05).Incidence of adverse anesthesia reactions in the study group was lower than the control group,data comparison was statistically significant between groups (P<0.05).Conclusion Ultrasound assisted femoral nerve block combined with infiltration anesthesia can achieve significant effect for elderly patients with total knee arthroplasty.
