摘　　要：目的探讨可靶向miR-146a的两个长链非编码RNA(lncRNA)NR_003367、NR_002819在原发性痛风性关节炎患者外周血单个核细胞(PBMCs)中的表达及其可能的临床意义。方法收集符合研究条件的60例痛风患者的临床资料及外周静脉血标本,其中包括急性期(AG)和间歇期(IG)各30例,另将50例健康体检者(健康对照组)的临床资料及外周静脉血标本作为对照组;采用逆转录-定量聚合酶链式反应(RT-qPCR)方法,检测两个长链非编码RNA(lncRNA)在痛风组和健康对照组PBMCs中的表达水平。比较这两个lncRNA在不同组别中的表达量差异,并进行与临床指标的相关性分析。使用受试者工作特征(ROC)曲线评估LncRNAs在痛风诊断中的效能。结果①NR_003367在痛风组的表达高于健康对照组(2.30×10^(-3)、1.10×10^(-3),Z=-3.65,P<0.01),进一步亚组分析显示AG组与IG组的表达均高于健康对照组(1.80×10^(-3)、2.60×10^(-3)与1.10×10^(-3);H=23.06;P<0.01),但AG组与IG组之间差异无统计学意义(P>0.05)。NR_002819在痛风组和健康对照组之间的表达差异无统计学意义(P>0.05)。②Spearman相关性分析发现痛风患者中NR_003367与甘油三酯(TG)呈正相关(r=0.31,P=0.02)。③NR_003367用于诊断痛风ROC曲线下面积为0.75。结论NR_003367在痛风中存在异常表达,且与部分临床指标相关,提示NR_003367可能参与了痛风发病,而NR_002819暂未被发现在痛风性关节炎中的作用。Objective To investigate the expression of two long non-coding RNAs(lncRNAs),NR_003367 and NR_002819,which can target miR-146a,in peripheral blood mononuclear cells(PBMCs)of patients with primary gouty arthritis and their possible clinical significance.Methods Clinical data and peripheral venous blood specimens of 60 gout patients eligible for the study were collected,including 30 in the acute phase(AG)and 30 in the intermittent phase(IG).Additionally,50 healthy controls(HC)were included.Reverse transcription-quantitative polymerase chain reaction(RT-qPCR)was used to measure the expression levels of NR_003367 and NR_002819 in PBMCs from both gouty arthritis and HC groups.Differences in the expression of these two lncRNA in different groups were compared,and correlation analysis with clinical indicators was performed.The efficacy of lncRNAs in the diagnosis of gout was assessed using a subject work characterization(ROC)curve.Results①The expression of NR_003367 was higher in the gouty arthritis group than that in the HC group(2.30×10^(-3),1.10×10^(-3),Z=-3.65,P<0.01),and further subgroup analyses showed that the expression was higher in both the AG and IG groups than that in the HC group(1.80×10^(-3),2.60×10^(-3)and 1.10×10^(-3);H=23.06;P<0.01),but the difference between AG and IG was not statistically significant(P>0.05).The difference in the expression of NR_002819 between gouty arthritis and HC groups was not statistically significant(P>0.05).②Spearman correlation analysis demonstrated that NR_003367 was positively correlated with triglycerides(TG)in gout patients(r=0.31,P=0.02).③NR_003367 was used to diagnose gout with an area under the ROC curve of 0.75.Conclusion NR_003367 is abnormally expressed in gout and correlates with certain clinical parameters,suggesting its potential involvement in the pathogenesis of gout.In contrast,NR_002819 does not appear to play a significant role in GA.
