摘 要:目的探讨全身振动训练(WBVT)结合神经肌肉训练治疗髌股疼痛综合征(PFPS)的效果。方法前瞻性选取2021年5月至2023年5月深圳市职业病防治院PFPS患者116例,按照随机数字表法分为2组,各58例。对照组给予神经肌肉训练,研究组给予WBVT结合神经肌肉训练,两组均干预12周。比较干预前、干预4周、干预12周后膝关节疼痛程度[视觉模拟评分(VAS)]、膝关节功能[膝前痛量表(AKPS)评分、Lysholm评分]、膝关节平衡能力[Y平衡测试(YBT)]、膝关节伸屈肌力[角速度60°/s、120°/s股四头肌峰力矩(PT)、腘绳肌PT]、生活质量[生活质量综合评定问卷-74(GQOL-74)]评分。结果研究组干预4周、12周后上下楼梯、跳跃时VAS评分低于对照组(P<0.05);研究组干预4周、12周后AKPS评分、Lysholm评分较对照组升高(P<0.05);研究组干预12周后膝关节前、左后、右后YBT值高于对照组(P<0.05);研究组干预4周、12周后角速度60°/s、120°/s股四头肌PT、腘绳肌PT高于对照组(P<0.05);研究组干预12周后GQOL-74量表中躯体功能、心理功能、社会功能、物质生活评分较对照组升高(P<0.05)。结论WBVT结合神经肌肉训练能减轻PFPS患者疼痛程度,改善膝关节功能,提高肌肉力量及平衡能力,从而提高患者生活质量。Objective To explore the effect of whole-body vibration training(WBVT)combined with neuromuscular training on patellofemoral pain syndrome(PFPS).Methods A prospective study was conducted from May 2021 to May 2023 at the Shenzhen Occupational Disease Prevention and Treatment Hospital,involving 116 patients with PFPS.These patients were randomly divided into two groups,each with 58 cases,according to a random number table method.The control group received neuromuscular training,while the study group received WBVT combined with neuromuscular training.Both groups underwent an intervention for 12 weeks.The knee pain severity[visual analog scale(VAS)],knee function[ankylosing knee pain scale(AKPS)score,Lysholm score],knee balance ability[Y balance test(YBT)],knee extensor and flexor muscle strength[angular velocity 60°/s,120°/s quadriceps peak torque(PT),hamstring PT],and quality of life[general quality of life assessment questionnaire-74(GQOL-74)]scores were compared before intervention,at 4 weeks after intervention,and at 12 weeks after intervention.Results After 4 and 12 weeks of intervention,the VAS scores for the study group were lower than those of the control group when climbing stairs and jumping(P<0.05).The AKPS and Lysholm scores in the study group were significantly higher than those in the control group after 4 and 12 weeks of intervention(P<0.05).There was no significant difference in the YBT values of the anterior,left posterior,and right posterior knee joints between the two groups before and after 4 weeks of intervention(P>0.05).However,after 12 weeks of intervention,the YBT values of the anterior,left posterior,and right posterior knee joints in the study group were higher than those in the control group(P<0.05);the PTs of quadriceps and hamstrings at angular velocities of 60°/s and 120°/s in the study group were higher than those in the control group after 4 and 12 weeks of intervention(P<0.05);after 12 weeks of intervention,the scores for physical function,psychological function,social function,a
