摘　　要：目的分析慢性心力衰竭患者血清可溶性细胞间黏附分子1(sICAM-1)、沉默信息调节因子2相关酶1(SIRT1)、半乳糖凝集素-3(Galectin-3)水平与心功能分级的关系及对预后的预测价值。方法回顾性纳入2020年6月至2022年6月我院收治的150例慢性心力衰竭患者为心力衰竭组,另选择同期健康体检者167例纳入健康对照组。慢性心力衰竭患者随访6个月,观察患者主要心脏不良事件发生情况。分析两组、不同心功能分级、不同预后慢性心力衰竭患者血清sICAM-1、SIRT1、Galectin-3水平,绘制受试者工作特征(ROC)曲线分析sICAM-1、SIRT1、Galectin-3单独及联合检测对预后的预测价值。结果与健康对照组比较,慢性心力衰竭组血清SIRT1水平更低,血清sICAM-1、Galectin-3水平更高(P<0.05)。心功能分级2级、3级、4级的慢性心力衰竭患者血清SIRT1水平逐渐降低,sICAM-1、Galectin-3水平呈升高趋势(P<0.05)。与预后良好组比较,预后不良组血清SIRT1水平更低,血清sICAM-1、Galectin-3水平更高(P<0.05)。绘制ROC曲线分析血清sICAM-1、SIRT1、Galectin-3联合检测对慢性心力衰竭患者预后不良的预测价值最高,AUC为0.946,灵敏度为93.48%、特异度为88.46%。结论随着慢性心力衰竭发生患者血清sICAM-1、Galectin-3水平均提高,SIRT1水平降低,且其与心功能分级增加、预后不良密切相关,联合检测以上指标患者预后不良的预测价值更高,可能作为慢性心力衰竭患者临床诊治的靶点。Objective To analyze relationship between serum soluble intercellular adhesion molecule 1(sICAM-1),silence-regulating factor 2 related enzyme 1(SIRT1),galactose lectin 3(Galectin-3)and cardiac function grade in patients with chronic heart failure and their prognostic value.Methods From June 2020 to June 2022,150 patients with chronic heart failure were included as chronic heart failure group,and 167 healthy subjects were included as healthy control group.Clinical data were retrospectively collected.Patients with chronic heart failure were followed up for 6 months to observe the occurrence of major cardiac adverse events.Serum sICAM-1,SIRT1 and Galectin-3 were analyzed between the two groups and with different cardiac function grades and different prognosis of chronic heart failure.Predictive values of sICAM-1,SIRT1 and Galectin-3 alone and in combination were analyzed by receiver operating characteristic(ROC)curve.Results Compared with healthy control group,serum SIRT1 in chronic heart failure group were lower,and sICAM-1 and Galectin-3 were higher(P<0.05).SIRT1 of patients with chronic heart failure in grade 2,3 and 4 showed a decreased trend,while sICAM-1 and Galectin-3 showed an increased trend(P<0.05).Compared with good prognosis group,serum SIRT1 in the poor prognosis group were lower,and sICAM-1 and Galectin-3 were higher(P<0.05).ROC curve was plotted to determine AUC of sICAM-1,SIRT1 and Galectin-3 combination for poor prognosis of patients with chronic heart failure were the highest AUC was 0.946,sensitivity was 93.48%,specificity was 88.46%.Conclusion Serum sICAM-1 and Galectin-3 increased and SIRT1 decreased in patients with chronic heart failure,which were closely related to increased cardiac function grade and poor prognosis,combined detection of the above indicators had higher predictive value of poor prognosis in patients,it might be a target for clinical diagnosis and treatment of patients with chronic heart failure.
