虚汗停颗粒联合右旋糖酐铁治疗儿童缺铁性贫血随机对照研究

摘　　要：目的:评估虚汗停颗粒联合右旋糖酐铁治疗儿童缺铁性贫血的有效性和安全性。方法:选取2022年4月至2023年11月于广州医科大学附属妇女儿童医疗中心诊治的72例缺铁性贫血患儿,按随机数表法分为观察组和对照组各36例。对照组仅接受右旋糖酐铁口服溶液治疗,观察组在对照组治疗基础上加用虚汗停颗粒,疗程均为4周。比较两组患儿治疗总有效率、红细胞参数、血清铁代谢指标、中医症状评分和不良反应发生率。结果:治疗4周后,观察组总有效率为94.4%,高于对照组的72.2%(P<0.05)。观察组的红细胞(RBC)、血红蛋白(HGB)、平均红细胞血红蛋白(MCH)、血清铁(SI)、铁蛋白(SF)水平均高于对照组(P均<0.05),血清总铁结合力(TIBC)低于对照组(P<0.05)。观察组中医症状(纳差、多汗、精神疲倦等)评分均低于对照组(P均<0.05)。两组患儿不良反应发生率比较差异无统计学意义(P>0.05)。结论:虚汗停颗粒与右旋糖酐铁的联合使用在治疗儿童缺铁性贫血方面显示出更高的治疗总有效率,可有效改善患儿的血液学和血清铁代谢指标,减轻相关中医症状,疗效优于单用右旋糖酐铁,且安全性良好。Objective:To evaluate the efficacy and safety of Xuhanting granules combined with iron dextran in the treatment of children with iron deficiency anemia.Methods:A total of 72 children with iron deficiency anemia who were diagnosed and treated at Women and Children’s Medical Center,Guangzhou Medical University from Apr.2022 to Nov.2023 were randomly divided into the control group and observation group via the random number table method,with 36 cases in each group.The control group was treated with iron dextran oral solution,while the observation group received Xuhanting granules on the basis of the control group.The treatment course was 4 weeks.Clinical effective rate,red blood cell parameters,serum iron metabolism parameters,traditional Chinese medicine(TCM)symptom score and incidence of adverse drug reactions were compared between two groups.Results:After 4 weeks of treatment,the total clinical effective rate was 94.4%in the observation group,significantly higher than 72.2%in the control group(P<0.05).The red blood cell(RBC),hemoglobin(HGB)and mean hemoglobin(MCH),serum iron(SI)and ferritin(SF)levels in the observation group were significantly higher than those in the control group(P<0.05),and the serum total iron binding capacity(TIBC)level was significantly lower than that in the control group (P<0.05). The scores of TCM symptom (anorexia, hyperhidrosis, and mental fatigue) in the observation group were significantly lower than those in the control group (P<0.05). There was no significant difference in the incidence of adverse drug reactions between two groups (P>0.05). Conclusion: The efficacy of Xuhanting granules combined with iron dextran in the treatment of children with iron deficiency anemia is more significant than iron dextran alone, which can effectively improve hematological and serum iron metabolism indicators and alleviate TCM related symptoms with higher safety.
