基于中医体质学说探讨特发性早发性脊柱侧弯的发病机制

摘　　要：基于中医体质学说探讨儿童体质类型与特发性早发性脊柱侧弯(idiopathic early-onset scoliosis,IEOS)发病的相关性。认为小儿体质不同,对IEOS易感性不同;同为IEOS患儿,但因其体质不同而分型不同,因此结合患儿体质特征、IEOS的辨证分型,探讨IEOS的发病与否及证候类型与体质的相关性。治疗当注重内调体质合柔筋松肉理骨的综合治疗思路,采取针、推、药三法并举治疗。内服中药补益脾肾,外在背部膀胱经和督脉进行推拿针灸以柔筋松肉理骨,按揉背俞穴以激发小儿脏腑及全身气血、自我调节能力,恢复脊柱的正常生理曲度。以期为中医学治疗IEOS提供新的思路。This paper explores the correlation between children's constitutional types and the etiology and pathogenesis of idiopathic early-onset scoliosis(IEOS)based on the constitution theory of traditional Chinese medicine(TCM).It is believed that children with different constitutions have varying susceptibilities to IEOS.Moreover,even among children diagnosed with IEOS,different constitutional types lead to different classifications.Therefore,the correlation between the occurrence of IEOS,syndrome types,and constitutional types is explored by combining the constitutional characteristics of IEOS patients with the syndrome differentiation of IEOS.Treatment should focus on internally regulating the constitution and comprehensively managing the condition through reasonable methods to soften muscles,relax tissues,and align bones.A combined approach of acupuncture,Tuina(massage therapy),and herbal medicine is adopted.Herbal medicines are administered internally to nourish the spleen and kidney,while acupuncture and Tuina are applied externally along the Bladder Meridian of Foot-Taiyang and Governor Vessel to soften muscles,relax tissues,and align bones.Tuina techniques are used on the back shu-points to stimulate the viscera,promote qi and blood circulation throughout the body,enhance self-regulatory abilities,and restore the normal physiological curvature of the spine.The aim is to provide new insights into the treatment of IEOS within the framework of TCM.
