摘 要:桑杲教授认为小儿厌食症的发生发展是一个逐步进展的过程,初期多因外邪损伤脾胃,致使饮食内停,而见食欲减退;病至中期,脾胃损伤加重,引起脾胃气虚或脾胃阴虚,中焦受损,纳化失司,则见不思饮食;后期脾胃亏虚日久,气血生化乏源,机体失于濡养,故除厌食外,还有虚证纷现。临床论治本病,桑教授常从“和稳论”出发,依据病程阶段不同分别施治:初期以和正祛邪为主,重在解除脾胃困境;中期以和稳脏腑为先,重在固护脾胃根本;后期以和调气血为主,除助运脾胃外,亦重视整体调节。此外,桑教授指出五脏之中肝脾关系尤为密切,故治疗小儿厌食症,亦需注意肝的调治。常用加味异功散加减,方证契合,收效显著。附验案1则,以资验证。Professor SANG Gao believes that the development of pediatric anorexia is a gradual process.In the early stage,it is often caused by external pathogens damaging the spleen and stomach,leading to food retention and reduced appetite.In the middle stage,the damage to the spleen and stomach worsens,resulting in spleen-stomach qi deficiency or spleen-stomach yin deficiency,which impairs the middle jiao and disrupts digestion,manifesting as a lack of desire to eat.In the late stage,prolonged deficiency of the spleen and stomach leads to insufficient production of qi and blood,causing malnutrition of the body,and in addition to anorexia,various deficiency symptoms appear.In clinical treatment,professor SANG approaches the disease from the perspective of the“harmony and stability theory,”applying different treatment strategies according to the disease stage:in the early stage,the focus is on harmonizing the body and expelling pathogens,primarily relieving the spleen and stomach from distress;in the middle stage,the priority is to stabilize the internal organs,emphasizing the protection of the spleen and stomach;in the late stage,the focus shifts to harmonizing qi and blood,not only aiding the spleen and stomach but also ensuring overall balance.Additionally,professor SANG highlights the close relationship between the liver and spleen among the five organs,emphasizing the importance of regulating the liver in treating pediatric anorexia.He commonly uses modified Jiawei Yigong San(加味异功散),which aligns well with the syndrome and yields significant results.A case is included to illustrate the effectiveness of this approach.
