摘 要:心房颤动(AF)是一种常见的心律失常性疾病,患病风险随着年龄增长而增加。AF的发生机制复杂,涉及心房结构重构(如心房纤维化、心肌细胞肥大等)、电重构(如离子通道异常、缝隙连接蛋白功能障碍等)及其他特殊重构(如功能重构、免疫重构等)。自噬的平衡对维持细胞稳态至关重要,其过度激活或抑制均可促进AF发展。自噬水平的异常可促进心房结构重构和电重构,而影响自噬通量的因素主要包括年龄、内质网应激、炎症及代谢紊乱。作为一种特殊的自噬形式,线粒体自噬在AF的形成和发展过程中也起着重要作用。针对自噬过程的干预可能为AF治疗开辟新途径,如4-苯基丁酸钠盐可抑制内质网应激相关的自噬,槲皮素可抑制心肌纤维化并改善心房重构。深入研究自噬与AF发生和发展的关系,对揭示AF发病机制和探索新的治疗策略具有重要意义。Atrial fibrillation(AF),a prevalent cardiac arrhythmia,exhibits a propensity for increased incidence with advancing age.The mechanism of AF is complex,involving atrial structural remodeling,which include atrial fibrosis and cardiomyocyte hypertrophy,alongside electrical remodeling characterized by ion channel abnormalities and connexin dysfunction.Additionally,the mechanism involves other distinct forms of remodeling,such as functional remodeling and immune remodeling.The balance of autophagy is crucial for cellular homeostasis,and its overactivation or inhibition may promote the development of AF.Disrupted autophagy contributes to atrial remodeling,influenced by age,endoplasmic reticulum stress,inflammation,and metabolic disorders.Mitophagy,a specific type of autophagy,plays an important role in the formation and development of AF.Targeting autophagy may lead to new treatments for AF.For instance,4-PBA can suppress autophagy linked to ER stress,while quercetin can prevent myocardial fibrosis and improve atrial remodeling.Exploring the link between autophagy and AF is crucial for understanding its causes and developing new treatments.
