布比卡因椎管内麻醉对老年骨科手术患者的麻醉效果及安全性评价

摘　　要：目的:探析布比卡因椎管内麻醉在老年骨科手术患者中的应用效果。方法:选取2023年9—12月钟山浩康医院骨科收治的70例老年手术患者作为研究对象,依据随机数字表法分为对照组(35例,全身麻醉)和观察组(35例,布比卡因椎管内麻醉)。对比两组认知功能、认知电位、血流动力学指标及并发症发生率。结果:术后1、6 h,两组简易精神状态评价量表评分、P3波幅参数均降低,观察组高于对照组(P<0.05);手术开始即刻、手术结束即刻,观察组心率和平均动脉压均高于对照组(P<0.05);观察组并发症发生率低于对照组(P=0.040)。结论:在老年骨科手术患者中,应用布比卡因椎管内麻醉可稳定血流动力学指标,改善术后认知功能,麻醉安全性较高。Objective:To explore the application effect of bupivacaine intraspinal anesthesia in elderly patients undergoing orthopedic surgery.Methods:Seventy elderly surgical patients admitted to the department of orthopedics of Zhongshan Haokang Hospital from September to December 2023 were selected as the research objects.According to the random number table method,they were divided into the control group(35 cases,general anesthesia)and the observation group(35 cases,spinal anesthesia with bupivacaine).The cognitive function,cognitive potential,hemodynamic indicators and the incidence of complications were compared between the two groups.Results:At 1 and 6 hours after the operation,the scores of the mini-mental state assessment scale and the P3 amplitude parameter in both groups decreased,and those in the observation group were higher than those in the control group(P<0.05).Immediately after the operation began and immediately after the operation ended,the heart rate and mean arterial pressure in the observation group were both higher than those in the control group(P<0.05).The incidence of complications in the observation group was lower than that in the control group(P=0.040).Conclusion:Among elderly patients undergoing orthopedic surgery,the application of bupivacaine intraspinal anesthesia can stabilize hemodynamic indicators,improve postoperative cognitive function,and has a relatively high anesthesia safety.
