组合式输尿管软镜治疗上尿路结石的临床效果

摘　　要：目的:探讨组合式输尿管软镜治疗上尿路结石的临床效果。方法:选取2023年1月—2024年7月安顺市镇宁布依族苗族自治县人民医院泌尿外科收治的上尿路结石患者60例为观察对象,随机双盲法分为参照组(30例,予以微创经皮肾镜联合钬激光碎石术治疗)和观察组(30例,予以组合式输尿管软镜联合钬激光碎石术治疗)。比较两组一次性碎石成功率、手术指标及并发症发生情况。结果:观察组一次性碎石成功率高于参照组(P=0.023);两组术中出血量比较,差异无统计学意义(P>0.05);观察组碎石时间、术后住院时间短于参照组(P<0.05);观察组并发症总发生率低于参照组(P=0.044)。结论:采用组合式输尿管软镜钬激光碎石术治疗上尿路结石患者,可提高一次性碎石成功率,缩短碎石时间、术后住院时间,降低并发症发生率。Objective:To explore the clinical effect of combined flexible ureteroscopy in the treatment of upper urinary tract calculi.Methods:Sixty patients with upper urinary tract calculi admitted to the department of urology of People's Hospital of Anshun Zhenning Buyi and Miao Autonomous County People's Hospital from January 2023 to July 2024 were selected as the observation objects.They were randomly and double-blind divided into the reference group(30 cases,treated with minimally invasive percutaneous nephroscopy combined with holmium laser lithotripsy)and the observation group(30 cases,treated with combined flexible ureteroscopy combined with holmium laser lithotripsy).The success rate of one-time lithotripsy,surgical indicators and the occurrence of complications were compared between the two groups.Results:The success rate of one-time lithotripsy in the observation group was higher than that in the reference group(P=0.023).There was no statistically significant difference in the intraoperative blood loss between the two groups(P>0.05).The lithotripsy time and postoperative hospital stay in the observation group were shorter than those in the reference group(P<0.05).The total incidence of complications in the observation group was lower than that in the reference group(P=0.044).Conclusion:The use of combined flexible ureteroscopy holmium laser lithotripsy for patients with upper urinary tract calculi can increase the success rate of one-time lithotripsy,shorten the lithotripsy time and postoperative hospital stay,and reduce the incidence of complications.
