超声引导下高位髂筋膜阻滞联合轻比重脊髓麻醉在老年髋部骨折患者手术中的应用效果

摘　　要：目的:评估超声引导下高位髂筋膜阻滞联合轻比重脊髓麻醉在老年髋部骨折患者手术中的应用效果。方法:选取2022年1月—2023年12月皖南医学院第二附属医院收治的72例老年髋部骨折患者为研究对象,随机分为对照组与观察组,各36例。对照组实施单侧轻比重脊髓麻醉,观察组实施超声引导下的高位髂筋膜阻滞联合轻比重脊髓麻醉。对比两组麻醉完全起效时间、麻醉效果、麻醉满意度、并发症发生情况。结果:观察组麻醉完全起效时间短于对照组(P<0.001);观察组Bromage评分及麻醉满意度高于对照组(P<0.05);两组并发症总发生率对比,无统计学差异(P>0.05)。结论:超声引导下高位髂筋膜阻滞联合轻比重脊髓麻醉在老年髋部骨折患者手术中的应用效果显著,麻醉起效快、效果好,患者满意度高,且未增加并发症发生风险。Objective:To evaluate the application effect of ultrasound-guided high iliac fascia block combined with light specific gravity spinal anesthesia in the surgery of elderly patients with hip fractures.Methods:A total of 72 elderly patients with hip fractures admitted to the Second Affiliated Hospital of Wannan Medical College from January 2022 to December 2023 were selected as the research objects and randomly divided into a control group and an observation group,with 36 cases in each group.The control group received unilateral light specific gravity spinal anesthesia,while the observation group received ultrasoundguided high iliac fascia block combined with light specific gravity spinal anesthesia.The complete onset time of anesthesia,anesthesia effect,anesthesia satisfaction and the occurrence of complications were compared between the two groups.Result:The complete onset time of anesthesia in the observation group was shorter than that in the control group(P<0.001).The Bromage score and anesthesia satisfaction in the observation group were higher than those in the control group(P<0.05).There was no statistically significant difference in the total incidence of complications between the two groups(P>0.05).Conclusion:The application effect of ultrasound-guided high iliac fascia block combined with light specific gravity spinal anesthesia in the surgery of elderly patients with hip fractures is significant.The anesthesia takes effect quickly,has a good effect,high patient satisfaction,and does not increase the risk of complications.
