内镜下腰椎间盘切除术及椎管扩大减压术治疗腰椎椎管狭窄症的效果研究

摘　　要：目的:分析内镜下腰椎间盘切除术及椎管扩大减压术治疗腰椎椎管狭窄症的效果。方法:选取德州市宁津县人民医院骨科2022年5月—2023年4月收治的腰椎椎管狭窄症患者86例开展研究,随机分为参照组和研究组,各43例。参照组接受开放性腰椎间盘切除术及椎管扩大减压术,研究组接受内镜下腰椎间盘切除术及椎管扩大减压术。比较两组手术相关指标、腰椎功能及疼痛程度。结果:研究组术后卧床时间短于参照组,手术切口长度、术中出血量、术后引流量低于参照组(P<0.001);术后1、3个月,两组Oswestry功能障碍评估量表评分低于术前,日本骨科协会评估表评分高于术前,研究组优于参照组(P<0.001);术后1、3个月,两组视觉模拟评分法评分低于术前,研究组低于参照组(P<0.001)。结论:内镜下腰椎间盘切除术及椎管扩大减压术的切口较小,能够降低术中出血量、术后引流量,加快术后恢复,改善腰椎功能,缓解疼痛。Objective:To analyze the effect of endoscopic lumbar discectomy and spinal canal dilated decompression surgery in the treatment of lumbar spinal stenosis.Methods:A total of eighty-six patients with lumbar spinal stenosis admitted to the department of orthopedics of Dezhou Ningjin County People's Hospital from May 2022 to April 2023 were selected for the study and randomly divided into the reference group and the study group,with forty-three cases in each group.The reference group received open lumbar discectomy and spinal canal dilated decompression surgery,while the study group received endoscopic lumbar discectomy and spinal canal dilated decompression.The surgery-related indicators,lumbar spine function and pain degree of the two groups were compared.Results:The postoperative bed rest time of the study group was shorter than that of the reference group,and the length of the surgical incision,intraoperative blood loss and postoperative drainage volume were lower than those of the reference group(P<0.001).One and three months after the operation,the scores of the Oswestry Disability Assessment Scale in both groups were lower than those before the operation,the scores of the Japanese Orthopaedic Association Assessment form were higher than those before the operation,and the study group was superior to the reference group(P<0.001).One and three months after the operation,the scores of the Visual Analogue Scale in both groups were lower than those before the operation,and the score of the study group was lower than that of the reference group(P<0.001).Conclusion:The incision of endoscopic lumbar discectomy and spinal canal dilated decompression surgery is relatively small,which can reduce intraoperative blood loss,postoperative drainage volume,accelerate postoperative recovery,improve lumbar function and relieve pain.
