基于中医辨证探讨慢性肾脏病相关瘙痒诊治进展

摘　　要：该文重点分析慢性肾脏病相关瘙痒的诊断、临床表现及中医辨证治疗策略,为解决慢性肾脏病相关瘙痒难题提供治疗思路。中医将慢性肾脏病相关瘙痒归为“风瘙痒”范畴,常见证候类型包括血虚风燥证、风湿热蕴肤证、风邪瘀毒证等,针对不同证型辨证治以养血润燥、祛风止痒;清热凉血、疏风止痒,疏风清热、利湿解毒止痒;开凑和营,润燥泻浊等,常用方剂涉及当归饮子、消风散、麻黄连翘赤小豆汤、麻黄桂枝各半汤等,经辨证用药,疗效确切。This article focuses on analyzing the diagnosis,clinical manifestations,and traditional Chinese medicine syndrome differentiation-based treatment strategies for chronic kidney disease-associated pruritus,providing therapeutic insights to address this challenging condition.In traditional Chinese medicine theory,chronic kidney disease-associated pruritus is categorized under"wind-induced pruritus",with common syndrome patterns including blood deficiency with wind-dryness,wind-dampness-heat accumulation in the skin,and wind-evil with blood stasis and toxin.Corresponding treatments are applied based on syndrome differentiation:nourishing blood and moistening dryness to dispel wind and relieve itching;clearing heat and cooling blood to disperse wind and stop itching;dispersing wind and clearing heat to eliminate dampness,detoxify,and relieve itching;as well as opening the interstices,harmonizing nutrient-defense levels,moistening dryness,and purging turbidity.Classic prescriptions such as Danggui Yinzi,Xiaofeng San,Mahuang Lianqiao Chixiaodou Tang,and Mahuang Guizhi Geban Tang are commonly used with proven efficacy when applied according to syndrome differentiation.
