摘　　要：急性心力衰竭是重症监护中的常见疾病,脱机失败是影响患者恢复的重要因素,该综述探讨有创动脉血压与甘油三酯-葡萄糖指数(TyG指数)在急性心力衰竭患者脱机过程中的应用。有创动脉血压监测能够实时评估血流动力学状态,为脱机决策提供重要依据。TyG指数作为胰岛素抵抗的代谢标志物,与心力衰竭的临床预后密切相关,其升高与脱机失败风险增加有关。有创动脉血压与TyG指数可以为脱机过程提供参考依据,提高脱机成功率,建议临床采用综合性评估方法,以优化急性心力衰竭患者的脱机管理并改善预后。Acute heart failure is a common disease in intensive care,and off-line failure is an important factor affecting patient recovery.This review explores the application of invasive arterial blood pressure and triglyceride-glucose index(TyG index)in offline patients with acute heart failure.Invasive arterial blood pressure monitoring can evaluate the hemodynamic status in real time and provide an important basis for off-line decision-making.As a metabolic marker of insulin resistance,TyG index is closely related to the clinical prognosis of heart failure,and its elevation is associated with an increased risk of offline failure.Invasive arterial blood pressure and TyG index can provide reference for the off-line process and improve the success rate of off-line.It is suggested that a comprehensive evaluation method should be adopted in clinic to optimize the off-line management and improve the prognosis of patients with acute heart failure.
