摘 要:目的探讨全员生产维护(TPM)联合6S[整理(seiri)、整顿(seiton)、清扫(seiso)、清洁(seiketsu)、素养(shitsuke)、安全(security)]管理法在骨科治疗仪器管理中的应用效果。方法选取医院骨科的18台治疗仪器作为研究对象,于2023年1—6月采用常规管理(管理前),于2023年7—12月采用TPM联合6S管理法(管理后),比较管理前后的仪器故障发生情况(包括仪器老化故障、机械故障、软件故障、电源故障、总故障率及故障处理周期)、仪器性能(包括仪器质量稳定性、时间利用性、诊疗有效性)、仪器日常管理情况(包括仪器规范放置率、登记完整率、规范操作率和清洁合格率)、仪器运行质量(包括仪器开机率、仪器功能完好率、仪器运转率)。结果实施TPM联合6S管理法后,仪器老化故障、机械故障、总故障率均低于管理前,故障处理周期长于管理前,仪器质量稳定性、时间利用性及诊疗有效性评分均高于管理前(P<0.05);仪器规范放置率、登记完整率、规范操作率、清洁优良率均高于管理前,仪器开机率、仪器功能完好率、仪器运转率均高于管理前,差异有统计学意义(P<0.05)。结论TPM联合6S管理法可以减少骨科治疗仪器的故障发生率,提高仪器的性能、运行稳定性及管理质量。Objective To explore the application effect of total productive maintenance(TPM)combined with 6S(seiri,seiton,seiso,seiketsu,shitsuke,and security)management method in the management of orthopaedic treatment instruments.Methods 18 therapeutic instruments in the orthopaedic department of the hospital were selected as the study object,and routine management(before management)was adopted from January to June 2023,and TPM combined with the 6S management method(after management)was adopted from July to December 2023,to compare the occurrence of instrument failures before and after the management(including instrument aging failures,mechanical failures,software failure,mechanical failure,software failure,power supply failure,total failure rate and failure treatment cycle),instrument performance(including instrument quality stability,time utilisation,diagnosis and treatment effectiveness),daily management of instruments(including the rate of standardised placement of instruments,the rate of complete registration,the rate of standardised operation and the rate of qualified cleanliness),and the quality of operation of instruments(including the rate of instrument start-up,the rate of intact function of instruments,and the rate of operation of instruments).Results After TPM combined with 6S management,the instrument aging failure,mechanical failure,and total failure rate were lower than before management,the fault handling cycle was longer than before management,the instrument quality stability,time utilisation,and diagnostic and treatment effectiveness scores were higher than before management(P<0.05);the rate of standardised placement of instruments,the rate of complete registration,the rate of standardised operation,and the rate of good cleanliness were all higher than before management,and the rate of instrument start-up,the rate of intact instrument function,and the rate of instrument operation were all higher than before management,and the difference was statistically significant(P<0.05).Conclusion TPM combined with 6S
