老年急性阑尾炎术后患者早期康复锻炼体验的质性研究

摘　　要：目的探讨老年急性阑尾炎术后患者早期康复锻炼的体验,为制订相关护理干预策略提供依据。方法采用目的抽样法,选取2023年1—7月贵州医科大学附属医院收治的老年急性阑尾炎术后患者作为调查对象,进行半结构式访谈。通过质性研究进行比较、分析和推理,采用Colaizzi 7步分析法形成主题及范畴,并进行完整陈述。结果提炼出4个主题:(1)早期康复锻炼行为建立困难;(2)早期康复锻炼积极与消极体验并存;(3)早期康复锻炼支持系统薄弱;(4)早期康复锻炼需求多样化。结论老年急性阑尾炎术后患者对早期康复锻炼持有积极态度,但个人差异和需求会影响康复锻炼的选择和行为。临床医护人员应重视患者的锻炼体验和需求,以促进早期康复锻炼的实施和提高康复效果。Objective To explore the experiences of early rehabilitation exercises in elderly patients with acute appendicitis after operation,and to provide evidence for formulating relevant nursing intervention strategies.Methods The purposive sampling method was used to select elderly patients who underwent acute appendectomy in the Affiliated Hospital of Guizhou Medical University from January to July 2023 as the subjects for semi-structured interviews.Qualitative research methods were used for comparison,analysis,and reasoning,and Colaizzi's 7-step analysis method was used to form themes and categories,and then a comprehensive description was made.Results Four themes were extracted:(1)difficulties in establishing early rehabilitation exercise behavior;(2)coexistence of positive and negative experiences in early rehabilitation exercises;(3)weak support system for early rehabilitation exercises;(4)diversified needs for early rehabilitation exercises.Conclusions Elderly patients after acute appendectomy have positive attitudes toward early rehabilitation exercises,but individual differences and needs will influence choices and behaviors of rehabilitation exercises.Clinical medical staff should pay attention to patients'exercise experiences and needs in order to facilitate the implementation of early rehabilitation exercises and improve rehabilitation effects.
