盐酸倍他司汀联合手法复位治疗良性阵发性位置性眩晕的临床效果

摘　　要：目的探讨盐酸倍他司汀联合手法复位治疗良性阵发性位置性眩晕(BPPV)的临床效果。方法选取2023年1月—2024年6月新疆生产建设兵团第四师医院收治的92例BPPV患者,按照不同治疗方法分为对照组与观察组各46例。对照组单用手法复位治疗,观察组在手法复位的基础上加用盐酸倍他司汀片口服治疗。比较两组的临床疗效、眩晕及平衡能力评分、脑血流动力学指标及复发率。结果观察组治疗总有效率高于对照组(P<0.05);前庭症状指数、眩晕残障评定量表评分低于对照组(P<0.05),Berg平衡量表评分高于对照组(P<0.05);左侧椎动脉、右侧椎动脉、基底动脉平均血流速度均高于对照组(P<0.05);治疗后3、6个月的复发率低于对照组(P<0.05)。结论盐酸倍他司汀联合手法复位治疗BPPV临床效果显著,能有效增加椎基底动脉供血,缓解眩晕症状,提高生活质量,降低复发率。Objective To investigate the clinical effect of betahistine hydrochloride combined with manual reduction in the treatment of benign paroxysmal positional vertigo(BPPV).Methods A total of 92 BPPV patients admitted to the Fourth Division Hospital of Xinjiang Production and Construction Corps from January 2023 to June 2024 were divided into control group and observation group with 46 cases in each group according to different treatment methods.The control group was treated with manual reduction alone,and the observation group was treated with betahistine hydrochloride tablets on the basis of manual reduction.The clinical efficacy,vertigo and balance score,cerebral hemodynamic index and recurrence rate were compared between the two groups.Results The total effective rate of observation group was higher than that of control group(P<0.05).Vestibular Symptom Index and Dizziness Handicap Inventory were lower than control group(P<0.05),but Berg Balance Scale was higher than control group(P<0.05).The average blood flow velocity of left vertebral artery,right vertebral artery and basilar artery was higher than that of control group(P<0.05).The recurrence rate of 3 and 6 months after treatment was lower than that of control group(P<0.05).Conclusions Betahistine hydrochloride combined with manual reduction in the treatment of BPPV has significant clinical effect,which can effectively increase the vertebrobasilar artery blood supply,relieve vertigo symptoms,improve quality of life,and reduce the recurrence rate.
