摘　　要：目的探讨改良B-lynch缝合法联合缩宫素治疗剖宫产产后出血(PPH)的临床效果。方法选取2021年5月—2023年5月新泰市人民医院收治的120例剖宫产PPH患者,采用随机数字表法分为对照组和观察组各60例。对照组予以缩宫素治疗,观察组采用改良B-lynch缝合法联合缩宫素治疗。比较两组出血量、产后恢复情况、血清指标、凝血指标及并发症发生率。结果观察组产后30 min、2 h、24 h出血量少于对照组(P<0.05);观察组血性恶露持续时间、下床活动时间及住院时间均短于对照组(P<0.05);产后24 h,观察组一氧化氮(NO)、一氧化氮合酶(NOS)、脑钠肽(BNP)水平均低于对照组(P<0.05),观察组凝血酶时间及活化部分凝血活酶时间短于对照组,纤维蛋白原水平高于对照组(P<0.05);两组患者术后并发症总发生率比较,差异无统计学意义(P>0.05)。结论改良B-lynch缝合法联合缩宫素治疗能够更有效地减少PPH患者的出血量,促进产后恢复,降低NO、NOS、BNP水平,并改善凝血指标,且不会增加并发症。Objective To investigate the effect of modified B-lynch suture combined with oxytocin in the treatment of postpartum hemorrhage(PPH)during cesarean section.Methods A total of 120 patients experiencing PPH during cesarean section at The People's Hospital of Xintai City from May 2021 to May 2023 were selected and randomly assigned into a control group and an observation group,each consisting of 60 cases.The control group was treated with oxytocin,and the observation group was treated with modified B-lynch suture combined with oxytocin.Blood loss,postpartum recovery,serum index,coagulation index and complication rate were compared between the two groups.Results The blood loss at 30 min,2 and 24 h postpartum in the observation group was less than that in the control group(P<0.05).The duration of lochia bleeding,the time to get out of bed,and the length of hospitalization in the observation group were shorter than those in the control group(P<0.05).At 24 h postpartum,nitric oxide(NO),nitric oxide synthase(NOS)and brain natriuretic peptide(BNP)in observation group were lower than those in control group(P<0.05);thrombin time and activated partial thrombin time in observation group were shorter than those in control group,and fibrinogen level was higher than that in control group(P<0.05).The comparison of the total incidence of postoperative complications between the two groups showed no statistically significant difference(P>0.05).Conclusions Modified B-lynch suture combined with oxytocin can more effectively reduce the amount of bleeding in PPH patients,promote postpartum recovery,reduce the level of NO,NOS,BNP,and improve coagulation index,without increasing complications.
