摘 要:目的探讨血清C肽与糖化血红蛋白联合检测对糖尿病的诊断价值。方法选取2022年3月—2023年10月莘县人民医院收治的160例疑似糖尿病患者,所有患者均检测血清C肽与糖化血红蛋白水平。统计患者的血清C肽和糖化血红蛋白阳性率,计算诊断的准确率、敏感度及特异度,并比较糖尿病患者与非糖尿病患者的血清C肽和糖化血红蛋白水平;绘制受试者工作特征(ROC)曲线,计算曲线下面积(AUC),评估血清C肽、糖化血红蛋白水平及联合检测对糖尿病的预测价值。结果以临床确诊结果为“金标准”,血清C肽诊断糖尿病的敏感度为70.00%,特异度为71.67%;糖化血红蛋白诊断的敏感度为77.00%,特异度为73.33%;联合检测诊断的敏感度为96.00%,特异度为73.33%;血清C肽及糖化血红蛋白二者联合诊断敏感度高于单一检测(P<0.05)。糖尿病患者的血清C肽水平低于非糖尿病患者,糖化血红蛋白水平高于非糖尿病患者(P<0.05)。绘制血清C肽、糖化血红蛋白水平及二者联合预测糖尿病的ROC曲线,结果显示,血清C肽、糖化血红蛋白水平及二者联合预测糖尿病发生的AUC分别为0.716、0.774、0.961,二者联合预测的AUC分别高于血清C肽、糖化血红蛋白,差异均有统计学意义(P<0.05)。结论血清C肽与糖化血红蛋白联合检测能够有效确诊糖尿病,准确判断病情,及时识别早期病变患者并鉴别病情进展,为制定针对性治疗方案提供依据。Objective To explore the diagnostic value of serum C-peptide combined with glycosylated hemoglobin in diabetes.Methods A total of 160 patients with suspected diabetes treated in Shenxian People's Hospital from March 2022 to October 2023 were selected.Serum C-peptide and glycosylated hemoglobin levels were detected in all patients.The positive rates of serum C-peptide and glycosylated hemoglobin were counted,the accuracy,sensitivity and specificity of diagnosis were calculated,the levels of serum C-peptide and glycosylated hemoglobin were compared between diabetic patients and non-diabetic patients,and receiver operating characteristic(ROC)curve was plotted and area under the curve(AUC)was calculated to evaluate the predictive value of serum C-peptide,glycated hemoglobin levels and combined detection for diabetes.Results"Gold standard"on the results of clinical diagnosis,the sensitivity of the serum C peptide in the diagnosis of diabetes was 70.00%,specific degree is 71.67%;Glycated hemoglobin diagnostic sensitivity of 77.00%,73.33%;The sensitivity and specificity of combined detection were 96.00%and 73.33%,respectively.The diagnostic sensitivity of the combination of serum C-peptide and glycosylated hemoglobin was higher than that of single test(P<0.05).The serum C-peptide level of diabetic patients was lower than that of non-diabetic patients,and the glycosylated hemoglobin level was higher than that of non-diabetic patients(P<0.05).Draw serum C peptide,glycated hemoglobin level and the combined prediction of diabetes ROC curves,the results showed that serum C peptide,glycated hemoglobin level and the two combined to predict diabetes AUC were 0.716,0.774,0.961,and the two combined prediction of AUC,respectively,higher than that of serum C peptide,glycosylated hemoglobin,The differences were statistically significant(P<0.05).Conclusions The combined detection of serum C-peptide and glycosylated hemoglobin can effectively diagnose diabetes mellitus,accurately judge the disease,timely identify early disease patient
