摘 要:目的探讨血清中三酰甘油(TG)对总蛋白(TP)检测产生影响的最低浓度范围,为临床TP的精准检测提供一定的参考依据。方法选取2023年1月—2024年5月南京大学医学院附属苏州医院检验科检测TG和TP的患者血清样本。按照样本的TG浓度分组,分别为1.70~2.29、2.30~5.69、5.70~11.29、11.30~24.91和24.92~40.42 mmol/L 5个组段。将每个样本等分为超速离心前样本与超速离心后样本,同时使用双缩脲法检测血清TP,所得结果进行统计学分析比较。结果血清TG浓度≥11.30 mmol/L的组段样本,在超速离心处理前后所检测的血清TP浓度差异CV均高于血清TG浓度<11.30 mmol/L的组段样本,差异有统计学意义(P<0.05)。结论用双缩脲法检测血清TP时,样本中所含TG干扰物质的最高浓度需<11.30 mmol/L。在实际工作中,检验工作者应对试剂盒说明书标注给出的干扰物质最低浓度值予以验证。当血清TG浓度≥11.30 mmol/L时,均应对样本血清进行超速离心预处理,尽可能消除TG后再做血清TP的检测。以免因TG的存在造成血清TP检测的假性增高,造成血清TP检验误差的发生。Objective To investigate the minimum threshold concentration of serum triglyceride(TG)that interferes with total protein(TP)measurement,providing reference data for accurate clinical TP testing.Methods Serum samples from patients undergoing both TG and TP tests at Department of Laboratory,the Affiliated Suzhou Hospital of Nanjing University Medical School between January 2023 and May 2024 were enrolled.Samples were stratified into five TG concentration groups:1.70-2.29,2.30-5.69,5.70-11.29,11.30-24.91,and 24.92-40.42 mmol/L.Each sample was equally divided into pre-ultracentrifugation and postultracentrifugation aliquots.TP levels were measured by biuret method,with comparative statistical analysis performed.Results In samples with TG≥11.30 mmol/L,the coefficient of variation(CV)of TP measurements before and after ultracentrifugation was significantly higher than in samples with TG<11.30 mmol/L(P<0.05).Conclusions When using biuret method for TP measurement,the maximum permissible TG interference concentration should be<11.30 mmol/L.Clinical laboratories should validate manufacturer-provided interference thresholds.For samples with TG≥11.30 mmol/L,ultracentrifugation pretreatment is recommended to eliminate TG interference and prevent falsely elevated TP results.
