成人结肠镜检查肠道准备质量的影响因素分析

摘　　要：目的调查新疆某三甲医院就诊者结肠镜下肠道清洁充分性的现状,并对影响因素进行探讨。方法采用自制问卷及波士顿肠道准备评分(BBPS)量表对370例成年人结肠镜受检者的肠道清洁质量展开调查,采用SPSS25.0软件,通过秩和检验以及多因素Logistic回归分析探究影响成人结肠镜检查准备质量的因素。结果370例成人患者中肠道清洁有效者328例(88.6%);了解肠道准备重要性、失眠、焦虑、紧张情绪、药物包装剂型的方便性、服药后出现头晕等不良反应症状及灌肠水温、灌肠前3天行清洁肠道预准备等6个问题是影响本次调查患者肠道清洁质量的危险因素。结论加强结肠镜检查前肠道准备健康宣教,关注患者术前情绪症状,用药后不良反应及检查前的肠道预准备指导,从而改善肠道准备清洁工作。Objective To investigate the current status of colonoscopic bowel cleansing adequacy in patients attending a tertiary hospital in Xinjiang,and to explore the influencing factors.Method A self-made questionnaire and the Boston Bowel Preparedness Score(BBPS)scale were used to survey the bowel cleansing quality of 370 adult patients undergoing colonoscopy.SPSS25.0 software was utilized to explore factors affecting the quality of preparation for adult colonoscopy examinations through rank sum tests and multivariate Logistic regression analysis.Results Among the 370 adult patients,328(88.6%)had effective bowel cleansing;six questions,such as understanding the importance of bowel preparation,insomnia,anxiety,nervousness,convenience of drug packaging dosage form,symptoms of adverse reactions such as dizziness after taking the drug and enema water temperature,and pre-preparation of clean bowel performed three days before enema,were the risk factors affecting the quality of bowel cleansing of the patients in this investigation.Conclusion Strengthening the health education of bowel preparation before colonoscopy,paying attention to the patients'preoperative emotional symptoms,adverse reactions after medication and the guidance of bowel pre-preparation before the examination,so as to improve the bowel preparation cleaning.
