艾滋病合并肺结核患者歧视知觉现状及对治疗依从性的影响

摘　　要：目的探讨艾滋病合并肺结核患者歧视知觉现状及对治疗依从性的影响。方法本研究为前瞻性研究,纳入2020年3月至2022年3月医院收治的108例艾滋病合并肺结核患者,均进行6个月治疗,分析不同治疗依从性患者的基线资料以及艾滋病合并肺结核患者歧视知觉对治疗依从性的影响。结果治疗6个月时,108例艾滋病合并肺结核患者整体治疗依从性处于中等水平;依从性不良组文化水平、婚姻状况、药物不良反应与依从性良好组比较(P<0.05);治疗前,108例艾滋病合并肺结核患者整体处于中等歧视知觉水平;依从性不良组SSS评分高于依从性良好组(P<0.05);经双变量Pearson相关性分析,艾滋病合并肺结核患者SSS评分与MMAS-8评分呈负相关(r=-0.756,P<0.001);经Logistic回归分析,婚姻状况、文化水平、药物不良反应、歧视知觉水平是艾滋病合并肺结核患者治疗依从性的影响因素(OR≠1,P<0.05)。结论艾滋病合并肺结核患者整体歧视知觉处于中等水平,且治疗依从性与歧视知觉水平有关。Objective To explore the status quo of discrimination perception in AIDS patients with pulmonary tuberculosis and its impact on treatment compliance.Methods This study was a prospective study.108 patients with AIDS and pulmonary tuberculosis admitted to the hospital from March 2020 to March 2022 were included in this study.All patients were treated for 6 months.The baseline data of patients with different treatment compliance and the impact of discrimination perception of AIDS patients with pulmonary tuberculosis on treatment compliance were analyzed.Results After 6 months of treatment,the overall treatment compliance of 108 AIDS patients with pulmonary tuberculosis was in the middle level;Comparison of cultural level,marital status,adverse drug reactions between poor compliance group and good compliance group(P<0.05).Before treatment,108 cases of AIDS patients with pulmonary tuberculosis were in the middle discrimination perception level as a whole.The SSS score of poor compliance group was higher than good compliance group(P<0.05).By bivariate Pearson correlation analysis,the SSS score of AIDS patients with pulmonary tuberculosis was negatively correlated with the MMAS-8 score(r=-0.756,P<0.001).Logistic regression analysis showed that marital status,educational level,adverse drug reactions and discrimination perception were the influencing factors of treatment compliance of AIDS patients with pulmonary tuberculosis(OR≠1,P<0.05).Conclusion The overall discrimination perception of AIDS patients with pulmonary tuberculosis is in the middle level,and treatment compliance is related to the discrimination perception level.
