摘　　要：目的探讨阿芬太尼联合丙泊酚对无痛纤维支气管镜检查的麻醉效果。方法选取该院2022年1月—2023年12月收治的150例无痛纤维支气管镜检查术的患者作为样本,根据麻醉诱导药物不同分为对照组(75例)和观察组(75例),对比两组采用不同麻醉诱导药物的患者在麻醉前1min、患者意识消失时、支气管镜进入声带、检查开始后3min、检查结束、拔除喉罩(前至后分别记为T0、T1、T2、T3、T4、T5)的平均动脉压、心率和脉搏血氧饱和度,记录患者的诱导期呛咳情况、不良事件发生率和麻醉药物停止后的患者恢复情况。结果在T1-T5时,观察组患者的MAP更高,T1-T3时观察组的HR明显高于对照组(P<0.05);观察组的诱导期呛咳情况和不良事件发生率均低于对照组(P<0.05);观察组的麻醉药物停止后的患者恢复时间均短于对照组(P<0.05)。结论阿芬太尼复合丙泊酚用于无痛纤维支气管镜检查可以减少诱导期呛咳及恶心呕吐和不良事件发生率,使血流动力学更加平稳、患者术后恢复期缩短。Objective To investigate the anesthetic effect of alfentanil combined with propofol on painless fiberoptic bronchoscopy.Methods A total of 150 patients who underwent painless fiber bronchoscopy from January 2022 to December 2023 were chosen as samples,and they were divided into control group(75 cases)and observation group(75 cases)based on different anesthesia induction drugs,The mean arterial pressure,heart rate,and satuation of pulse oximetry between both groups were compared with those of patients treated with different anesthesia induction drugs at various time points:1 minute before anesthesia,when the patient's consciousness was lost,during bronchoscopy as it entered the vocal cords,3 minutes after the examination began,and 3 minutes after the laryngeal mask was removed(T0,T1,T2,T3,T4,T5,respectively).The patient's choking during the induction period,the incidence of adverse events,and the patient's recovery after the discontinuation of anesthetic drugs were recorded.Results At T1-T5,the MAP of the observation group was higher,and the HR of the observation group was significantly higher than that of the control group at T1-T3(P<0.05).The incidence of cough and adverse events in the observation group was lower than that in the control group(P<0.05).The recovery time of patients in the observation group after the cessation of anesthetic drugs was shorter than that in the control group(P<0.05).Conclusion The use of alfentanil combined with propofol in painless fiber bronchoscopy can reduce the incidence of choking,nausea and vomiting and adverse events during the induction period,make the hemodynamics more stable,and shorten the postoperative recovery period of patients.
