摘　　要：目的探讨衰弱状态与老年(年龄≥60岁)冠脉搭桥术(CABG)术后患者预后相关性。方法选择2020年4月~2021年4月在本院心脏外科就诊的老年冠心病行冠脉搭桥患者共442例,所有患者于搭桥前均进行老年综合评估,根据结果将患者分为衰弱组240例和对照组202例。对入组患者自搭桥后进行为期1年随访,终点事件定义为心血管不良事件。结果随访结果显示,与对照组比较,衰弱组患者心血管不良事件发生率明显增高[43例(17.9%)比21例(10.4%),P<0.05]。通过多因素logistic回归分析,去除年龄等混杂因素后,衰弱状态仍是老年CABG术后患者1年后发生心血管不良终点事件的独立预测因素(P<0.05)。结论衰弱状态为老年CABG术后患者的不良预后的独立危险因素,故对需进行搭桥手术的老年患者进行衰弱状态评估对患者的危险分层级预后判断具有一定意义。Objective To investigate the relationship between Frailty and prognosis in elderly patients with coronary heart disease(aged≥60)undergoing coronary artery bypass surgery(CABG).Methods 442 elderly patients with coronary heart disease and undergoing CABG who were treated at the department of cardiac surgery of People's Hospital of Xinjiang Uygur Autonomous Region from April 2020 to April 2021 were selected.All patients were assessed for Frailty within 24 hours after admission,and followed up for 1 year after discharge by outpatient and telephone.Incidence of major adverse cardiac events during 1 year was analyzed.Results The percentage of major adverse cardiac events in the Frailty group were significantly higher than that in the control group(P<0.05).Multivariate logistic regression analyses revealed that the Frailty was an independent risk factor for cardiovascular endpoint events in elderly patients undergoing CABG within one year after discharge(P<0.05).Conclusion Frailty is correlated with the prognosis of elderly patients undergoing CABG,and the assessment of Frailty in elderly patients undergoing CABG is important in making treatment plan and prognosis assessment.
