摘　　要：目的探讨肌肉骨骼超声联合血清IL-6在RA疾病活动度中的应用价值。方法收集289例既往在新疆维吾尔自治区人民医院风湿免疫科住院的RA患者。收集患者的关节肿胀数、关节压痛数、红细胞沉降率(ESR)、C反应蛋白(CRP)、血清IL-6及双腕、双手第1-5掌指关节(MCP)、第1指间关节,第2-5近端指间关节(PIP)共22个关节肌肉骨骼超声。结果(1)RA疾病活动度指标DAS28-ESR与滑膜增生(Grey Scale GS)评分总和(r=0.156,P=0.008),滑膜炎(Power Doppler,PD)评分总和(r=0.248,P=0.000),关节腔积液总和(r=0.269,P=0.000),腱鞘炎评分总和(r=0.204,P=0.000)呈正相关,但与骨侵蚀(r=0.115,P=0.05)不相关;(2)DAS28-ESR与血清IL-6呈正相关(r=0.135,P=0.011),DAS28-CRP与血清IL-6呈正相关(r=0.151,P=0.005);(3)血清IL-6与关节腔积液总和(r=0.206,P=0.000),GS评分总和(r=0.147,P=0.012),PD评分总和(r=0.184,P=0.002),腱鞘炎评分总和(r=0.145,P=0.014)呈正相关,但与骨侵蚀(r=0.112,P=0.05)不相关。结论肌肉骨骼超声联合血清IL-6评估病情,为RA患者提供更客观、直接的证据。Objective To investigate the value of musculoskeletal ultrasound(MSUS)with IL-6 in assessing the disease activity of rheumatoid arthritis(RA).Methods As a retrospective study,clinical data(swollen joints count,tender joints count),laboratory test(erythrocyte sedimentation rate,C-reaction protein,interleukin-6 IL-6)results and MSUS results of 22 joints were collected from 289 hospitalized RA patients in the The People's Hospital of the Xinjiang Uygur Autonomous Region.Result(1)There was positive correlation between DAS28-ESR and Grey Scale(GS)(r=0.156,P=0.008),Power Doppler(PD)(r=0.248,P=0.000),sum scores of articular cavity effusion(r=0.269,P=0.000),tenosynovitis(r=0.204,P=0.000)in RA patients,but there was no correlation with bone erosion(r=0.115,P=0.05).(2)There was positive correlation between DAS28-ESR and IL-6(r=0.135,P=0.011),there was positive correlation between DAS28-CRP and IL-6(r=0.151,P=0.005).(3)There was positive correlation between IL-6 and sum scores of articular cavity effusion(r=0.206,P=0.000),GS(r=0.147,P=0.012),PD(r=0.184,P=0.002),tenosynovitis(r=0.145,P=0.014),but there was no correlation with bone erosion(r=0.112,P=0.05).Conclusion The MUSU and IL-6 can be assessment the disease activity of RA patients,and provide a more direct and objective evidence for the rapid evaluation of disease.
