摘　　要：目的探讨HIV病毒感染者新冠感染临床特征影响和因素分析。方法收集405例HIV病毒感染者新冠感染的一般人口学资料、临床表现、临床分型、疫苗接种和诊治疗效等资料进行分析总结。结果405例患者中男性244例、女性161例,平均年龄为45.43±12.24岁,普通型116例、轻型283例、重型6例,无死亡病例。重型患者平均年龄高于轻型、普通型患者(P<0.01),轻型患者平均BMI低于普通型重型患者(P<0.01),重型组CD4细胞计数明显低于轻型、普通型患者(P<0.05)。频次统计显示HIV感染者新冠肺炎感染症状前5的有:发热、咽痛、咳嗽、乏力、肌痛。层次聚类结果显示,患者症状可分为三大主要症状群:发热、呼吸道主要表现的症状群(鼻塞、乏力、咳嗽、咽痛)、全身多系统症状群(咯血、气促、胃肠道症状、嗅觉味觉减退、肌痛、咳痰、头痛)。结论HIV感染新冠感染者主要症状群有发热、呼吸道主要表现的症状群、全身多系统症状群,患者以轻型、普通型为主,少数患者可发展成重型,不同临床分型的主要影响因素有:年龄、BMI、CD4细胞计数。Objective To investigate the clinical characteristics and influencing factors of COVID-19 infection in HIV-infected patients.Methods Data on general demographics,clinical manifestations,clinical classifications,vaccination status,and treatment outcomes were collected and analyzed from 405 HIV-infected patients with COVID-19.Results Among 405 patients,there were 244males and 161 females,with a mean age of 45.43±12.24 years.There were 116 moderate cases,283 mild cases,and 6 severe cases,with no deaths.Severe cases had a significantly higher mean age compared to mild and moderate cases(P<0.01),mild cases had significantly lower mean BMI compared to moderate and severe cases(P<0.01),and the severe group had significantly lower CD4 cell counts than mild and moderate cases(P<0.05).Frequency statistics showed the top 5 symptoms in HIV-infected patients with COVID-19 were fever,sore throat,cough,fatigue,and myalgia.Hierarchical clustering analysis revealed that three major symptom clusters were fever,primary respiratory symptoms(nasal congestion,fatigue,cough,sore throat),and systemic multi-system symptoms(hemoptysis,shortness of breath,gastrointestinal symptoms,reduced smell and taste,myalgia,sputum,headache).Conclusion The main symptoms for HIV-infected individuals who also have COVID-19 include fever,respiratory symptoms,and systemic symptoms affecting multiple systems.Most patients present with mild or moderate cases,while a minority may progress to severe cases.The primary factors influencing different clinical classifications are age,BMI,and CD4 cell count.
