摘  要:目的 探讨EB病毒抗体检测联合外周血白细胞形态在儿童上呼吸道感染中的应用价值。方法 选取2022年7月-2024年6月抚州市妇幼保健院收治的50例EB病毒致儿童上呼吸道感染者为观察组,另选同期来院体检的健康儿童50例为对照组。均开展EB病毒抗体、外周血白细胞形态检查。统计上呼吸道感染检出情况及诊断价值,并采用kappa检验不同方法与金标准的一致性。结果 共选取100例儿童,其中EB病毒致儿童上呼吸道感染50例,EB病毒抗体检查检出44例,检出率为88.00%;外周血白细胞形态检出43例,检出率为86.00%;联合检查检出49例,检出率为98.00%;联合检查诊断的灵敏度、准确度高于两种单一检查,阴性预测值高于外周血白细胞形态检查,有统计学差异(P<0.05);kappa检验显示,EB病毒抗体、外周血白细胞形态、联合检查与金标准的kappa值分别为0.800、0.740、0.940,联合检查一致性最高。结论 EB病毒抗体联合外周血白细胞形态检查可提高儿童上呼吸道感染诊断准确性,便于早期及时治疗,保障患儿健康成长。Objective To explore the application value of EB virus antibody detection combined with peripheral blood leukocyte morphology in children with upper respiratory tract infections.Methods 50 children with upper respiratory tract infections caused by EB virus admitted to Fuzhou Maternal and Child Health Hospital from July 2022 to June 2024 were selected as the observation group,and 50 healthy children who came to the hospital for physical examination during the same period were selected as the control group.Conduct EB virus antibody and peripheral blood leukocyte morphology tests.Statistically analyze the detection status and diagnostic value of upper respiratory tract infections,and use Kappa test to determine the consistency between different methods and the gold standard.Results A total of 100 children were selected,including 50 cases of upper respiratory tract infection caused by EB virus,and 44 cases were detected by EB virus antibody examination,with a detection rate of 88.00%;43 cases of peripheral blood white blood cell morphology were detected,with a detection rate of 86.00%;49 cases were detected through joint examination,with a detection rate of 98.00%;the sensitivity and accuracy of joint examination diagnosis are higher than those of two single examinations,and the negative predictive value is higher than that of peripheral blood leukocyte morphology examination,with statistical differences(P<0.05);the kappa test showed that the EB virus antibody,peripheral blood leukocyte morphology,and kappa values of the combined test and gold standard were 0.800,0.740,and 0.940,respectively,indicating the highest consistency in the combined test.Conclusion the combination of EB virus antibodies and peripheral blood leukocyte morphology examination can improve the diagnostic accuracy of upper respiratory tract infections in children,facilitate early and timely treatment,and ensure the healthy growth of children.
