基于“肠-关节”轴探讨艾灸对膝骨性关节炎模型兔肠道菌群的影响

摘  要:目的 观察艾灸对膝骨性关节炎模型兔肠道菌群的影响,探究其治疗膝骨性关节炎的作用机制。方法 将24只6月龄、体重3.5 kg左右雄性新西兰兔随机分为正常组、模型组和艾灸组,每组8只。除正常组外,其余兔均注射4%木瓜蛋白酶溶液制备膝骨性关节炎模型。造模成功次日,艾灸组进行艾灸干预,每日1次,每次40 min,持续2周。对比干预前后膝关节软骨病理变化、血清炎症因子水平、肠道菌群变化。结果 艾灸组改善了膝骨性关节炎模型兔的关节软骨结构紊乱,降低白介素-1β、肿瘤坏死因子-α和IL-8水平(均P<0.05)。16S rDNA测序结果显示,模型组中颤螺菌属和克雷伯菌属为优势菌属,而艾灸组草酸杆菌属为优势菌属。艾灸显著提高了拟杆菌门的相对丰度(P<0.05),显著降低了厚壁菌门/拟杆菌门(F/B)比值(P<0.05)。结论 艾灸能有效保护关节软骨、减轻炎症、延缓膝骨性关节炎的进展。其机制可能与调节肠道菌群结构有关,其中F/B比值和草酸杆菌属起到关键调节作用。Objective To observe the effects of moxibustion on the gut microbiota of rabbits with knee osteoarthritis(KOA).Methods Twenty-four rabbits were randomly divided into normal control group,model group and moxibustion group(n=8 each group).Except the normal group,KOA models were prepared in all the other groups by injection of 4%papain solution.The moxibustion group received moxibustion intervention every day for 40 minutes for 2 weeks.The pathological changes in the cartilage,serum inflammatory factors and intestinal microecology before and after intervention were compared among groups.Results Moxibustion improved the disorder in articular cartilage structure and reduced the levels of interleukin(IL)-1β,tumor necrosis factor-a(TNF-α),and IL-8(all P<0.05).16S rDNA results showed that Oscillospira and Klebsiella were dominant bacterial genera in model group,while Oxalobacter was the dominant bacterial genus in moxibustion group.Moxibustion significantly increased the relative abundance of Bacteroidetes(P<0.05) and decreased the Firmicutes/Bacteroidetes(F/B) ratio(P<0.05).Conclusion Moxibustion can effectively protect articular cartilage,reduce inflammation,and delay the progression of KOA.Its mechanism may be related to the adjustment of gut microbiota structure,with particular contribution to the regulation of F/B ratio and Oxalobacter.
