琥珀酰明胶电解质醋酸钠注射液用于外科手术容量治疗的药物经济学评价

摘　　要：目的:评价琥珀酰明胶电解质醋酸钠注射液相较于琥珀酰明胶注射液用于外科手术中需要容量治疗患者的经济性。方法:分别使用成本效果分析(CEA)和成本效用分析(CUA),从中国卫生体系视角出发,比较琥珀酰明胶电解质醋酸钠注射液和琥珀酰明胶注射液用于外科手术中需要容量治疗患者的健康产出和成本产出,并对关键参数进行敏感性分析。结果:CEA结果显示,以输注药品结束后15 min碱剩余较基线变化值为效果指标时,增量成本为21.23元,ICER值为18.30元/mmol·L^(-1),即碱剩余相较基线每少变化1个单位,需要多花费18.30元。CUA结果显示,琥珀酰明胶电解质醋酸钠注射液对比琥珀酰明胶注射液的增量成本为21.23元,增量质量调整生命年(QALYs)为0.001 0,增量成本效果比(ICER)值为21 193.36元/QALY,约为0.24倍2023年人均GDP(89 358元)。结论:在外科手术中对需要容量治疗的患者,使用琥珀酰明胶电解质醋酸钠注射液对保持人体内环境稳定的效果优于琥珀酰明胶注射液;且和琥珀酰明胶注射液相比,琥珀酰明胶电解质醋酸钠注射液更具成本效用,是一种十分经济的治疗方案。OBJECTIVE To evaluate the economics of succinylated gelatin,multiple electrolytes and sodium acetate injection(SG-MESAI)versus succinylated gelatin injection in patients requiring surgical volume therapy.METHODS From the perspective of Chinese healthcare system,the health outcomes and economic outcomes in patients requiring surgical volume therapy using SG-MESAI versus succinylated gelatin injection were compared using cost-effectiveness analysis(CEA)and cost-utility analysis(CUA).Key parameters were assessed by the sensitivity analysis.RESULTS CEA data showed an incremental cost of 21.23 yuan and an incremental cost-effectiveness ratio(ICER)of 18.30 yuan/mmol·L^(–1) when using the change in base excess(BE)between the baseline and 15 minutes of infusion as an efficacy indicator.CUA data showed that the incremental cost of SGMESAI versus succinylated gelatin injection was 21.23 yuan,with incremental quality-adjusted life years(QALYs)of 0.0010 and an ICER of 21193.36 yuan/QALY,which was approximately 0.24 times the 2023 GDP per capita(89358 yuan).CONCLUSION In patients requiring surgical volume therapy,the use of SG-MESAI is more effective than succinylated gelatin injection in maintaining a stable fluid environment and more cost-effective,making it a very economical treatment option.
