摘  要:间变性淋巴瘤激酶(anaplastic lymphoma kinase,ALK)抑制剂在治疗ALK基因突变非小细胞肺癌中具有良好的临床疗效。ALK抑制剂的体内药动学存在显著个体间差异,其体内暴露与安全性及有效性存在一定相关性。治疗药物监测(therapeutic drug monitoring,TDM)可以监测患者体内药物浓度变化,基于定量药理学理论建模从而实现治疗方案的优化。该文全面综述了ALK抑制剂的吸收、分布、代谢及排泄的药代动力学特征和相关影响因素,治疗药物监测的研究进展,以期为ALK抑制剂药物个体化用药提供参考。Anaplastic lymphoma kinase(ALK)inhibitors have demonstrated promising clinical efficacy in treating ALKmutant non-small cell lung cancer.In vivo pharmacokinetics of ALK inhibitors shows a significant individual disease,presenting a correlation of in vivo exposure with the safety and efficacy.Therapeutic drug monitoring(TDM)allows for timely tracking of drug concentration changes in patients,playing a vital role in optimizing treatment regimens based on the quantitative pharmacological theoretical modeling.This review discussed the pharmacokinetic characteristics of ALK inhibitors,including the absorption,distribution,metabolism and excretion,and influencing factors.Meanwhile,we also described the research progress in TDM of ALK inhibitors,aiming to provide a reference for the individualized use of ALK inhibitors.
