摘 要:目的比较低年资超声医师应用国际卵巢肿瘤分析组织(IOTA)简单法则与卵巢-附件影像报告和数据系统(O-RADS)对附件肿块良恶性的鉴别诊断价值。方法回顾性分析我院经病理证实的116例附件肿块患者(共144个附件肿块,良性123个,恶性21个)的超声图像资料,由2名低年资超声医师分别采用常规超声主观评估、IOTA简单法则、O-RADS判断附件肿块良恶性。以病理结果为金标准,绘制受试者工作特征曲线分析不同方法诊断附件肿块良恶性的效能。结果常规超声主观评估、IOTA简单法则、O-RADS诊断附件肿块良恶性的准确率分别为75.69%、89.58%、86.81%,灵敏度分别为76.19%、95.24%、90.48%,特异度分别为75.61%、88.62%、86.18%,曲线下面积分别为0.76、0.92、0.88;IOTA简单法则、O-RADS的曲线下面积均高于常规超声主观评估,差异均有统计学意义(均P<0.001)。结论应用IOTA简单法则、O-RADS均可较好地诊断附件肿块良恶性,其中IOTA简单法则的灵敏度较O-RADS更高,更易于低年资超声医师掌握和应用。Objective To compare the diagnostic value of the International Ovarian Tumor Analysis(IOTA)simple rules and the ovarian-adnexal reporting and data system(O-RADS)in differentiating benign and malignant adnexal masses when applied by junior sonographers.Methods Retrospective analysis was performed on ultrasound imaging data from 116 patients with pathologically confirmed adnexal masses(totally 144 masses,including 123 benign and 21 malignant).Two junior sonographers independently evaluated the benign and malignant adnexal masses using conventional ultrasound assessment,IOTA simple rules and O-RADS,respectively.Taking pathological results as the gold standard,receiver operating characteristic(ROC)curve was drawm to compare the diagnostic performance of the three methods.Results The diagnostic accuracies for conventional ultrasound assessment,IOTA simple rules,and O-RADS were 75.69%,89.58%and 86.81%,sensitivities were 76.19%,95.24%and 90.48%,specificities were 75.61%,88.62%and 86.18%,the area under the curve were 0.76,0.92 and 0.88,respectively,the area under the curve of IOTA simple rules and O-RADS were higher than that of conventional ultrasound assessment,with statistical significances(both P<0.001).Conclusion The application of IOTA simple rules and O-RADS can effectively differentiate benign and malignant adnexal masses,with the IOTA simple rules demonstrating higher sensitivity than that of O-RADS and being easier for junior sonographers to master and apply.
