中国老年人肌肉质量与失能关系的前瞻性队列研究

摘　　要：目的探索肌肉质量与老年人失能的关系。方法本研究基于中国老年健康影响因素跟踪调查项目2008—2018年四期的追踪调查数据,利用Cox回归,并构建限制性立方样条模型分析肌肉质量与老年人失能的剂量-反应关系。结果纳入2165名老年人,随访4次,平均随访10年。其中603名(27.8%)发展为失能老人;调整混杂因素后肌肉质量对老年人失能产生影响(HR=0.879,95%CI:0.822~0.939);RCS结果显示,老年人肌肉质量与失能风险的关联在老年男性(P_(总趋势)<0.05,P_(非线性)<0.05)和老年女性(P_(总趋势)<0.05,P_(非线性)<0.05)中均呈非线性增高趋势,提示老年男性ASMI应维持7.2(kg/m^(2))及以上水平和老年女性ASMI应维持5.2(kg/m^(2))及以上水平。结论老年人肌肉质量与其失能存在关联,通过维持或提高肌肉质量水平,可以改善老年人晚期的生活质量。Objective To explore the relationship between muscle mass and disability in the elderly.Methods This study is based on the tracking survey data from the Chinese Longitudinal Healthy Longevity Survey(CLHLS)from 2008 to 2018,utilizing Cox regression and constructing restricted cubic spline models to analyze the dose-response relationship between muscle mass and disability in the elderly.Results A total of 2165 elderly patients were followed up 4 times for an average of 10 years.Among them,603(27.8%)developed disability.After adjusting for confounders,muscle mass affected disability in the elderly(HR=0.879,95%CI:0.822-0.939).The RCS results showed that the association between muscle mass and disability risk in older adults was non-linear in both older men(P_(overall trend)<0.05,P_(non-linear)<0.05)and older women(P_(overall trend)<0.05,P_(non-linear)<0.05).It is suggested that ASMI level of 7.2(kg/m^(2))and above should be maintained in elderly men and 5.2(kg/m^(2))and above in elderly women.Conclusion Muscle mass is associated with disability in the elderly.Maintenance or improvement of muscle mass can improve the quality of life in the elderly.
