摘 要:目的探讨机械通气新生儿发生肺不张的影响因素,并构建风险预测模型。方法采用便利抽样法选取天津医科大学附属宝坻医院儿科于2021年1月-2024年1月收治的进行机械通气治疗的120例新生儿为研究对象。根据是否在机械通气治疗期间发生肺不张,分为肺不张组及未发生肺不张组。通过查阅医院电子病历系统,收集并自制调查表统计患儿性别、胎龄、出生体重等一般资料,采用单因素分析及多因素回归分析机械通气新生儿发生肺不张的影响因素,并构建风险预测模型。结果120例机械通气新生儿中,发生肺不张22例,占18.33%。胎龄、机械通气时间、有创通气时间、是否气管插管、吸入氧浓度、是否使用血管活性药物及呼吸系统是否感染为机械通气新生儿发生肺不张的影响因素(P<0.05);多因素分析结果显示,胎龄、是否气管插管、有创通气时间及呼吸系统是否感染为机械通气新生儿发生肺不张的独立危险因素(P<0.05)。ROC曲线下面积为0.976[95%CI(0.951,1.000)],表明该模型具有较强的预测能力。结论机械通气新生儿肺不张的发生风险较高,胎龄、有创通气时间、是否气管插管及呼吸系统是否感染为患儿发生肺不张的危险因素,基于以上危险因素构建的风险预测模型准确率较高,可为临床提供依据。医护人员应加以关注并采取针对性预防措施,从而减少机械通气新生儿肺不张的发生风险。Objective To explore the influencing factors of atelectasis in mechanically ventilated newborns and construct a risk prediction model.Methods Convenience sampling was used to select 120 newborns who underwent mechanical ventilation treatment in the pediatrics department of Baodi Hospital affiliated to Tianjin Medical University from January 2021 to January 2024 as the study subjects.According to whether atelectasis occurred during mechanical ventilation treatment,they were divided into atelectasis group and non atelectasis group.By consulting the hospital’s electronic medical record system,collecting and self-made survey forms to statistically analyze general information such as gender,gestational age,and birth weight of infants,single factor analysis and multiple regression analysis were used to identify the influencing factors of atelectasis in mechanically ventilated newborns,and a risk prediction model was constructed.Results Out of 120 mechanically ventilated newborns,22 cases(18.33%)experienced atelectasis.Gestational age,duration of mechanical ventilation,duration of invasive ventilation,presence of endotracheal intubation,inhaled oxygen concentration,use of vasoactive drugs,and respiratory system infection are factors that affect the occurrence of atelectasis in mechanically ventilated newborns(P<0.05);the results of multivariate analysis showed that gestational age,whether tracheal intubation was performed,duration of invasive ventilation,and respiratory system infection were independent risk factors for atelectasis in mechanically ventilated newborns(P<0.05).The area under the ROC curve is 0.976[95%CI(0.951,1.000)],indicating that the model has strong predictive ability.Conclusion Newborns undergoing mechanical ventilation have a higher risk of developing atelectasis,with gestational age,duration of invasive ventilation,presence of endotracheal intubation,and respiratory system infection being risk factors.The risk prediction model constructed based on these risk factors has a high accuracy and can pr
