摘　　要：慢性鼻窦炎(chronic rhinosinusitis,CRS)这一概念与中医学中“鼻渊”相对应。升清降浊理论起源于《内经》,汉代张仲景对该理论进行发挥并应用,清代医家李东垣对该理论进行了全面完善创新。国医大师干祖望临床诊治CRS重视全身气机,认为鼻窍作为头面五官之一,其位置结构特点,与肺、脾、肝胆诸脏腑经络的联系决定了清浊气机升降对于维持鼻窍诸功能的重要性,并指出清阳不升、浊阴不降是CRS的重要病机之一。文章概括总结干教授治肺经、调脾胃、清肝胆三法以达升清降浊之效,并附干祖望临床验案一则,以供同道探讨。The concept of chronic rhinosinusitis(CRS)corresponds to the“Sinusitis”in traditional Chinese medicine.The theory of ascending clearness and descending turbidity originated from Neijing,Zhang Zhongjing played and applied the theory in Han Dynasty,and Li Dongyuan,a doctor in Qing Dynasty,improved and innovated the theory comprehensively.Master ofTraditional Chinese Medicine Gan Zuwang attaches importance to the whole body's qi mechanism in the clinical diagnosis and treatment of CRS,and believes that as one of the five senses of the head and face,the positional and structural characteristics of the nostrils,and the connection with the lungs,spleen,liver and gallbladder organs and meridians determines the importance of the elevation of clear and turbid qi to maintain the functions of the nostrils,and points out that the failure of clear yang to elevate and the failure of turbid yin to descend is one of the important mechanisms for the illnesses of CRS.This paper summarizes Professor Gan's three methods of treating lung meridian,regulating spleen and stomach,and clearing liver and gallbladder to achieve the ascension of clearness and descending of turbidity,and attaches a clinical case of Gan Zuwang for the discussion of fellow practitioners.
