摘  要:目的本研究旨在比较三维相位对比磁共振静脉成像(3D-PC-MRV)中软组织图(FFE/M)、血管图(PCA/M)及PCA/M联合FFE/M对脑静脉窦血栓形成的诊断价值。方法本研究纳入26例234段静脉窦行磁共振MRV检查的患者,分为PCA/M、FFE/M及PCA/M联合FFE/M 3组,由2名资深影像诊断医师对各静脉窦疾病进行诊断,采用χ^(2)检验、交叉表及Kappa分析对结果进行统计学分析。结果3组方法数据差异有统计学意义(P<0.05),以造影检查为金标准,PCA/M联合FFE/M组脑静脉窦血栓形成灵敏度为97.1%,优于FFE/M组,差异有统计学意义;脑静脉窦狭窄灵敏度为91.7%,优于PCA/M组,差异有统计学意义;特异度为99.4%,均优于FFE/M组和PCA/M组,差异有统计学意义;且与造影检查为金标准的Kappa值为0.958,一致性较好。结论3D-PC-MRV检查PCA/M联合FFE/M的诊断效能优于单独评价PCA/M、FFE/M得到的诊断结果,可以作为颅内静脉磁共振成像的首选检查方法。Objective To compare the diagnostic value of soft tissue imaging(fully flow encoded modulus,FFE/M),vascular imaging(phase contrast angiography modulus,PCA/M),and the combination of PCA/M and FFE/M in three-dimensional phase-contrast magnetic resonance venography(3D-PC-MRV)for cerebral venous sinus thrombosis.Methods A total of 26 patients with 234 segments who underwent magnetic resonance venography were included and divided into three groups:PCA/M group,FFE/M group,and PCA/M combined with FFE/M group.Two experienced radiologists independently diagnosed the cerebral venous sinus disorders.Statistical analyses,includingχ^(2) test,cross-tabulation,and Kappa analysis,were performed to evaluate the results.Results There were statistically significant differences among the three groups(P<0.05).Using angiography as the gold standard,the sensitivity of the PCA/M combined with FFE/M group for detecting cerebral venous sinus thrombosis was 97.1%,which was higher than that of the FFE/M group,and the difference was statistically significant.The sensitivity for detecting cerebral venous sinus stenosis was 91.7%,which was higher than that of the PCA/M group,and the difference was statistically significant.The specificity was 99.4%for both groups,which was higher than that of the FFE/M group and the PCA/M group,and the differences were statistically significant.The Kappa value for agreement with the angiography gold standard was 0.958,indicating good consistency.Conclusion In intracranial venous magnetic resonance imaging,the diagnostic performance of the PCA/M combined with FFE/M approach in 3D-PC-MRV is superior to the individual evaluation of PCA/M and FFE/M,making it a preferred method for examination.
