摘  要:目的探究磁共振(MR)在胎儿脑组织结构畸形和中枢神经系统异常检查中的临床价值。方法对我院2023年1月至2023年12月135例行产前检查的孕妇临床资料做回顾性分析,均行MR检查。“金标准”为孕妇最终妊娠结果,对比不同检查方法下胎儿发育异常的检查结果,并对两者检查在胎儿发育异常中的诊断价值进行分析,计算该诊断结果与“金标准”的一致性。另分析MR检查胎儿异常发育类型,对比侧脑室增宽与中枢神经系统异常之间的关系。结果135例孕妇中经“金标准”检查明确检出胎儿脑组织结构畸形和中枢神经系统异常阳性24例,阳性检出率为17.8%(24/135)。超声检查检出阳性28例,阳性检出率为20.7%(28/135);MR检查检出阳性26例,阳性检出率为19.3%(26/135)。MR检查下诊断灵敏度、特异度、准确度、阳性及阴性预测值均高于超声检查,且一致性诊断也高于超声检查,差异有统计学意义(P<0.05)。不同类型胎儿发育异常在单、双侧及后角内径比较中差异有统计学意义(P<0.05)。结论MR在胎儿脑组织结构畸形和中枢神经系统异常鉴别诊断具有较高的临床应用价值,其诊断结果与“金标准”存在较强的一致性,同时可及时了解侧脑室增宽与中枢神经系统之间的关系,为临床进一步诊疗提供可靠的参考,具有广泛推广应用价值。Objective To explore the clinical value of magnetic resonance(MR)in the examination of fetal brain tissue structural abnormalities and central nervous system abnormalities.Methods A retrospective analysis was conducted on the clinical data of 135 pregnant women who underwent prenatal examinations in our hospital from January 2023 to December 2023,all of whom underwent MR examinations.The"gold standard"refers to the final pregnancy result of pregnant women,comparing the results of fetal developmental abnormalities under different examination methods,and analyzing the diagnostic value of the two examinations in fetal developmental abnormalities.The consistency between this diagnostic result and the"gold standard"was calculated.Furthermore,the types of fetal abnormal development detected by MR examination were analyzed and the relationship between lateral ventricular widening and central nervous system abnormalities was compared.Results Among 135 pregnant women,24 cases were confirmed positive for fetal brain tissue structural abnormalities and central nervous system abnormalities through the"gold standard"examination,with a positive detection rate of 17.8%(24/135).Twenty-eight cases were detected as positive by ultrasound examination,with a positive detection rate of 20.7%(28/135).Twenty-six cases were detected as positive by MR examination,with a positive detection rate of 19.3%(26/135).The diagnostic sensitivity,specificity,accuracy,and positive and negative predictive values of MR examination were higher than those of ultrasound examination,and the consistency of diagnosis was also higher than that of ultrasound examination,with statistical differences(P<0.05).There were statistically significant differences(P<0.05)in the comparison of single,bilateral,and posterior angular diameters among different types of fetal developmental abnormalities.Conclusion MR has high clinical application value in the differential diagnosis of fetal brain tissue structural abnormalities and central nervous system abnormalities,and i
