摘 要:目的:探究运动联合饮食干预对高血压患者的降压效果。方法:通过中国知网、万方、维普,以及Embase、PubMed、Web of Science、Cochrane Library等数据库,检索自建库起至2024年12月6日所有与运动联合饮食干预对高血压患者疗效相关的随机对照试验(RCT)。使用RevMan5.4软件对筛选后的6篇研究进行Meta分析,样本量总计771人。结果:与对照组(饮食干预)相比,干预组(运动联合饮食干预)的收缩压(SBP)和舒张压(DBP)有显著改善;干预时长达到6个月时,SBP和DBP的降低效果显著;干预2个月和4个月时血压未呈现出显著差异。对比有氧运动联合饮食干预与抗阻运动联合饮食干预,有氧运动联合饮食干预的降压效果显著;而抗阻运动联合饮食干预,仅对DBP的影响较为显著,对SBP的影响未显示出明显差异。结论:运动联合饮食干预比单纯饮食干预对血压控制更有利,有氧运动联合饮食干预比抗阻运动联合饮食干预在降压方面效果更为突出,且干预时长达到6个月时降压效果更佳。OBJECTIVE:This study is aimed to investigate the blood pressure-lowering effects of exercise-diet combined interventions in hypertensive patients.METHODS:Randomized controlled trials(RCTs)evaluating the efficacy of exercise-diet combined interventions for hypertension were retrieved from CNKI,Wanfang,VIP,Embase,PubMed,Web of Science,and Cochrane Library databases,spanning from their inception to December 6,2024.Six studies(total sample size:771 participants)were included for a meta-analysis using RevMan 5.4 software.RESULTS:Compared to the control group(diet-only intervention),the intervention group(exercise-diet combined intervention)showed significant improvements in systolic blood pressure(SBP)and diastolic blood pressure(DBP).A 6-month intervention duration demonstrated significant SBP and DBP reductions,whereas no significant differences were observed at 2-month and 4-month time points.Aerobic exercise combined with dietary intervention exhibited superior antihypertensive efficacy compared to resistance exercise combined with dietary intervention.In contrast,the resistance exercise combined with dietary intervention exhibits statistically significant efficacy only on DBP,while showing no clinically meaningful impact on SBP.CONCLUSION:Exercise-diet combined intervention demonstrates greater advantages for blood pressure control compared to diet-only intervention.Aerobic exercise combined with dietary interventions exhibits more pronounced blood pressure-lowering effects than resistance exercise combined with dietary interventions.Optimal outcomes require sustained implementation over 6 months.
