摘 要:目的:分析大孔径磁共振左侧卧位扫描检查对孕妇的舒适度及胎儿图像质量的影响;方法:选取厦门大学附属妇女儿童医院在2023年1—12月进行孕晚期孕检的孕妇50例作为研究对象,所有孕妇均行大孔径磁共振脑部成像检查,均采用左侧卧位和仰卧位扫描,对两种体位检查进行孕妇舒适度评分及胎儿影像各脑区指标进行测量并比较。结果:50例孕妇中,左侧卧位舒适度评分在3~5分,平均舒适度为(4.23±0.63)分,仰卧位舒适度评分在2~5分,平均舒适度为(3.23±0.44)分,差异有统计学意义(P<0.001)。左侧卧位和仰卧位胎儿成像质量主观评分、脑脊液清晰度得分比较,差异无统计学意义(P>0.05)。左侧卧位中左侧中脑、右侧小脑信噪比(SNR)值高于仰卧位,差异有统计学意义(P<0.05)。其余脑区两组差异无统计学意义(P>0.05)。左侧卧位中左侧枕叶对比噪声比(CNR)评分高于仰卧位,差异有统计学意义(P<0.05)。其余脑区两组差异无统计学意义(P>0.05)。结论:大孔径磁共振左侧卧位扫描相对于仰卧位在孕妇的舒适度和胎儿部分脑区图像质量方面具有优势,尤其在提高信噪比和噪声比方面表现更好,这些发现对于临床磁共振检查中选择合适体位以获得更好的胎儿图像质量和提高孕妇舒适度具有指导意义。Objective To analyze the effffects of left lateral decubitus magnetic resonance scan on the comfort of pregnant women and fetal imaging quality.Methods Fifty pregnant women in the third trimester of pregnancy in the Xiamen University Affiffiliated Women and Children's Hospital from January 2023 to December 2023 were selected as the study subjects.All pregnant women underwent large-aperture magnetic resonance imaging(MRI),and all of them were scanned in the left decubitus position and supine position.Comfort scores and indicators of each brain region of fetal imaging were measured in the two examination ways.Results Among the 50 pregnant women,the comfort score of the left lateral decubitus position was 3-5 points,and the average comfort score was(4.23±0.63)points;the comfort score of the supine position was 2-5 points,and the average comfort score was(3.23±0.44)points,and the difffference was statistically signifificant(P<0.001).There was no satistical difffference in the subjective scores of fetal imaging quality and cerebrospinal flfluid scores between left lateral decubitus and supine positions(P>0.05).The signal-to-noise ratio(SNR)values of left midbrain and right cerebellum in left lateral decubitus position were higher than those in supine position(P<0.05).There was no signifificant difffference in other brain regions between the two groups(P>0.05).The contrast-to-noise ratio(CNR)score of left occipital lobe was higher in left decumbent position than in supine position,and the difffference was statistically signifificant(P<0.05).There was no satistical difffference in other brain regions between the two groups(P>0.05).Conclusions Compared with supine position,left lateral decubitus decumbent MRI scan has advantages in the comfort and image quality of some brain regions of pregnant women,especially in improving the SNR and CNR ratio.These fifindings have guiding signifificance for selecting the appropriate position in clinical MRI to obtain better fetal image quality and improve the comfort of pregnant wom
