CT与MRI在老年多发性脑梗死中的诊断价值分析

摘　　要：目的:探究CT、MRI检查在老年多发性脑梗死患者中的诊断价值。方法:选取从2022年5月—2024年5月在齐河县人民医院就诊的疑似多发性脑梗死老年患者102例,均采取CT、MRI检查,以磁共振灌注加权成像(PWI)检查为金标准,比较CT、MRI平扫序列的诊断效果。结果:102例疑似患者中,金标准检出阳性68例、阴性34例。CT诊断检出真阳性57例,真阴性21例;MRI诊断检出真阳性66例,真阴性31例。CT诊断的阴性预测值为65.63%、特异度为61.76%、阳性预测值为81.43%、灵敏度为83.82%、准确率为76.47%,均低于MRI检查(P<0.05)。CT对丘脑、基底节区、颞叶、额叶、脑室旁等病灶的诊断符合率均较MRI诊断低(P<0.05)。结论:对疑似老年多发性脑梗死患者早期确诊,采取MRI诊断相比CT诊断,诊断效能更佳,可早期检出小缺血病灶。Objective To explore the diagnostic value of CT and MRI in elderly patients with multiple cerebral infarction.Methods A total of 102 elderly patients suspected of having multiple cerebral infarction,who were admitted to QihéCounty People’s Hospital from May 2022 to May 2024,underwent CT,and MRI.Using MRI PWI imaging as the gold standard,the diagnostic performance of CT and MRI was compared.Results Among the 102 suspected cases,the gold standard identifified 68 positive and 34 negative cases.CT diagnosis detected 57 true positives and 21 true negatives,while MRI diagnosis detected 66 true positives and 31 true negatives.The negative predictive value(NPV)of CT was 65.63%,specifificity was 61.76%,positive predictive value(PPV)was 81.43%,sensitivity was 83.82%,and accuracy was 76.47%,all of which were signifificantly lower than those of MRI(P<0.05).CT also showed lower diagnostic concordance rates compared to MRI for lesions in the thalamus,basal ganglia,temporal lobe,frontal lobe,and periventricular regions(P<0.05).Conclusions For early diagnosis of suspected multiple cerebral infarction in elderly patients,MRI demonstrates superior diagnostic effifficacy over CT,particularly in detecting small ischemic lesions at an early stage.
