摘 要:目的分析三维能量多普勒超声(3D-PDUS)与容积成像在宫腔粘连鉴别中的应用价值。方法回顾性分析60例疑似宫腔粘连患者的临床资料,所有患者均行3D-PDUS、容积成像及宫腔镜检查。以宫腔镜检查结果为“金标准”,将3D-PDUS、容积成像检查诊断结果与“金标准”进行一致性评价,分析3D-PDUS、容积成像检查诊断宫腔粘连的诊断价值。结果3D-PDUS、容积成像检查诊断宫腔粘连的准确度分别为95.00%(57/60)、81.97%(50/60),敏感度分别为95.35%(41/43)、79.55%(35/43),特异度分别为94.12%(16/17)、88.24%(15/17),阳性预测值分别为97.62%(41/42)、94.59%(35/37),阴性预测值分别为88.89%(16/18)、62.50%(15/24),Kappa值分别为0.879、0.602。3D-PDUS检查诊断宫腔粘连的准确度、敏感度高于容积成像检查诊断,差异有统计学意义(P<0.05);两种检查诊断的特异度比较,差异无统计学意义(P>0.05)。结论3D-PDUS检查诊断宫腔粘连准确度较高,可为临床诊断提供依据。Objective To analyze the application value of three-dimensional power Doppler ultrasound(3D-PDUS)and volume imaging in the differentiation of intrauterine adhesions.Methods The clinical data of 60 patients with suspected intrauterine adhesions were retrospectively analyzed.All patients underwent 3D-PDUS,volume imaging,and hysteroscopic examination.Taking the results of hysteroscopic examination as the“gold standard”,the diagnostic results of 3D-PDUS and volume imaging were evaluated for consistency with the“gold standard”,and the diagnostic value of 3D-PDUS and volume imaging in diagnosing intrauterine adhesions was analyzed.Results The accuracies of 3D-PDUS and volume imaging in diagnosing intrauterine adhesions were 95.00%(57/60)and 81.97%(50/60),respectively.The sensitivities were 95.35%(41/43)and 79.55%(35/43),respectively.The specificities were 94.12%(16/17)and 88.24%(15/17),respectively.The positive predictive values were 97.62%(41/42)and 94.59%(35/37),respectively.The negative predictive values were 88.89%(16/18)and 62.50%(15/24),respectively.The Kappa values were 0.879 and 0.602,respectively.The accuracy and sensitivity of 3D-PDUS in diagnosing intrauterine adhesions were higher than those of volume imaging,and the differences were statistically significant(P<0.05).There was no statistically significant difference in the specificity between the two diagnostic methods(P>0.05).Conclusion 3D-PDUS has a relatively high accuracy in diagnosing intrauterine adhesions and can provide a basis for clinical diagnosis.
